摘要
目的:探讨经鼻高流量氧疗对中重度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者呼吸参数及上颌窦气化程度的影响。方法:选取2020年6月—2022年3月在赣州市人民医院接受治疗的中重度OSAHS患者82例,按信封法随机分为对照组与观察组,各41例。两组均接受对症支持治疗,对照组给予持续气道正压通气治疗,观察组给予经鼻高流量氧疗治疗。比较两组临床疗效、呼吸参数[动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO_(2))、呼吸频率(RR)、氧饱和度下降指数(ODI)]、上颌窦气化程度及睡眠质量[匹兹堡睡眠质量指数(PSQI)]评分。结果:观察组治疗总有效率为95.12%,高于对照组的78.05%(P<0.05)。两组治疗后PaO_(2)、RR均较治疗前上升,且观察组均高于对照组,PaCO_(2)、ODI均较治疗前下降,且观察组均低于对照组(P<0.05)。两组治疗前后上颌窦前后径、左右径及上下径比较,差异均无统计学意义(P>0.05)。两组治疗后PSQI各维度评分较治疗前均下降,且观察组均低于对照组(P<0.05)。结论:经鼻高流量氧疗治疗中重度OSAHS患者效果理想,不仅能有效改善呼吸参数,还能提升睡眠质量,但对上颌窦气化程度无明显的作用。
Objective:To investigate the effects of high flow nasal therapy on respiratory parameters and gasification degree of maxillary sinus in patients with moderate to severe obstructive sleep apnea hypopnea syndrome(OSAHS).Method:A total of 82 patients with moderate to severe OSAHS who received treatment in Ganzhou People's Hospital from June 2020 to March 2022 were selected and randomly divided into control group and observation group according to envelope method,with 41 cases in each group.Both groups received symptomatic supportive treatment,the control group received continuous positive airway pressure treatment,and the observation group received nasal high flow nasal therapy.The clinical efficacy,respiratory parameters[(partial arterial oxygen pressure(PaO_(2)),partial pressure of carbon dioxide in artery(PaCO_(2)),respiratory rate(RR),oxygen desaturation index(ODI)],gasification degree of maxillary sinus and sleep quality[Pittsburgh sleep quality index(PSQI)]scores of the two groups were compared.Result:The total effective rate of observation group was 95.12%,which was higher than 78.05% of control group(P<0.05).After treatment,PaO_(2) and RR in both groups were higher than those before treatment,and those in the observation group were higher than those in the control group,PaCO_(2) and ODI in both groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).There were no significant differences in anterior and posterior diameter,left and right diameters and upper and lower diameters of maxillary sinus between the two groups before and after treatment(P>0.05).After treatment,the scores of PSQI in all dimensions of both groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).Conclusion:High flow nasal therapy has an ideal effect on patients with moderate to severe OSAHS,which can not only effectively improve respiratory parameters,but also improve sleep quality,but has no obvious effect on the gasification degree of maxillary sinus.
