摘要
目的观察自拟消肿方对原发性肾病综合征(膜性肾病)的临床疗效。方法将52例膜性肾病患者按照随机数字表法分为2组,均给予降脂及降压、利尿消肿的基础治疗,对照组加厄贝沙坦;治疗组在对照组基础上加自拟消肿方。2组均以12周为1个疗程,共治疗1个疗程。比较2组治疗前后证候积分变化;比较2组治疗前后尿素氮(BUN)、肌酐(Cr)、24 h尿蛋白定量,血白蛋白及血脂指标血清胆固醇(TC)变化。结果治疗组疗效优于对照组;2组治疗后周身水肿,泡沫尿,形寒肢冷,腰膝酸软,神疲乏力积分均较本组治疗前降低,且治疗组降低更明显;2组治疗后BUN、Cr、24 h尿蛋白定量、TC均较本组治疗前降低,且治疗组降低更明显;2组治疗后血白蛋白均较治疗前升高,且治疗组升高更明显(P<0.05)。结论自拟消肿方治疗原发性肾病综合征,能有效改善临床症状、减少尿蛋白、升高血白蛋白、降低血脂,效果显著。
Objective To observe the clinical effect of self-designed Xiaozhong prescription on primary nephrotic syndrome(membranous nephropathy).Methods 52 patients with membranous nephropathy were divided into two groups according to the random number table method.All patients were treated with atorvastatin calcium tablets for lipid-lowering,hypotensive,diuresis and swelling reduction.The control group was treated with irbesartan.The treatment group was treated with self-made Xiaozhong prescription on the basis of the control group.12 weeks was a course of treatment in both groups.The changes of syndrome score before and after treatment were compared between the two groups.The changes of renal function BUN,Cr,24h urine protein quantity,serum albumin and TC before and after treatment were compared between the two groups.Results The therapeutic effect of the treatment group was better than that of the control group.After treatment,the body edema,foam urine,cold shape and cold limbs,waist and knee pain and fatigue scores in the 2 groups were lower than before treatment,and the reduction was more obvious in the treatment group.BUN,Cr,24 h urinary protein quantity and TC in 2 groups after treatment were lower than before treatment,and the reduction was more obvious in the treatment group.The serum albumin of 2 groups after treatment was higher than that before treatment,and the increase of the treatment group was more obvious(P<0.05).Conclusion The self-made Xiaozhong prescription can effectively improve the clinical symptoms,reduce urinary protein,increase serum albumin,and reduce blood lipid in the treatment of primary nephrotic syndrome.
