摘要
目的为提高国家药品集中带量采购(简称国家集采)政策的实施效果提供参考。方法采用立意抽样方法,于2021年12月选取山西省17名长期从业于医药卫生领域且熟知国家集采政策的工作人员进行深度访谈。基于程序化扎根理论对访谈资料进行开放式编码、主轴编码、选择性编码,探讨国家集采政策对医药领域的影响。结果共逐级梳理出310个初始概念、108个初始范畴、27个副范畴、5个主范畴,并形成以政府部门为主导,以医疗保险、医疗机构、制药企业为枢纽,以老百姓为最终受益方的逻辑框架。结论国家药品集采政策有效促进了医保体制改革、卫生体制改革与药品流通体制改革三者的联动,大幅降低了老百姓的用药负担,促进了医药行业的正规化,是医药卫生体制改革取得成效的关键举措。但在如何提升基层医务人员的工作积极性,促进医保控费监管与临床合理用药的协同,引领制药企业参与市场的公平竞争,加大国家集采政策的宣传及普及等方面仍待进一步完善。
Objective To provide a reference for promoting the implementation of the national-centralized drug procurement(NCDP)policy.Methods Seventeen senior staff in the fields of medicine and health in Shanxi province and familiar with the NCDP policy were selected for the in-depth interview in December 2021 by the purposive sampling method.The open coding,axial coding and selective coding were performed on the interview data based on the programed grounded theory to investigate the effect of NCDP policy on the pharmaceutical industry.Results A total of 310 initial concepts,108 initial categories,27 sub-categories and five main categories were sorted out step by step,and a logical framework led by government departments,with medical insurance,medical institutions and pharmaceutical enterprises as the key factors,and with the common people as the ultimate beneficiaries was formed.Conclusion The NCDP policy effectively promotes the interaction of medical insurance system reform,health system reform and drug circulation system reform,greatly reduces the medication burden of common people,and promotes the standardization of the pharmaceutical industry,which is a key measure to improve the effectiveness of reform of the medicine and healthcare system.However,there are still some challenges that need to be further improved,such as how to improve the work enthusiasm of grass-roots medical staff,promote the coordination between supervision of medical insurance cost and clinical rational drug use,lead pharmaceutical enterprises to participate in fair competition in the market,and increase the publicity and popularity of NCDP policy.
