摘要
目的:探讨血清前肾上腺髓质素(pro-ADM)在呼吸机相关性肺炎(VAP)诊断中的作用。方法:选取2020年6月—2022年12月郓城县丁里长镇卫生院重症监护病房进行有效机械通气治疗、疑似VAP的患者68例,根据诊断结果分为VAP组、非VAP组,各34例。比较两组第1、3、5天pro-ADM、降钙素原(PCT)、C反应蛋白(CRP)、白细胞计数(WBC)以及临床肺部感染(CPIS)评分,评价pro-ADM对VAP的诊断价值。结果:第1、3、5天,VAP组血清pro-ADM、PCT、CPIS水平高于非VAP组,差异有统计学意义(P<0.05);第3、5天,VAP组血清CRP水平高于非VAP组,差异有统计学意义(P<0.05);第1、3天,VAP组血清WBC水平高于非VAP组,差异有统计学意义(P<0.05);两组第1天CRP水平、第5天WBC水平比较,差异无统计学意义(P>0.05);第1天血清pro-ADM水平敏感性与特异性最高,截断值为2.53 nmol/L。结论:pro-ADM对VAP诊断价值较高,在早期诊断中敏感性与特异性较高,可以用于VAP早期诊断。
Objective:To explore the role of serum pro-adrenomedullin(pro-ADM)in the diagnosis of ventilator-associated pneumonia(VAP).Methods:Sixty-eight patients who were treated with effective mechanical ventilation and suspected of VAP in the intensive care unit of Yuncheng County Dinglichang Town Health Center from June 2020 to December 2022 were select⁃ed and divided into VAP group and non-VAP group according to the diagnosis,each of which consisted of thirty-four cases.The diagnostic value of pro-ADM for VAP was evaluated by comparing pro-ADM,procalcitonin(PCT),C-reactive protein(CRP),white blood cell count(WBC),and clinical pulmonary infection(CPIS)scores on days 1,3,and 5 in the two groups.Re⁃sults:On days 1,3,and 5,serum pro-ADM,PCT,and CPIS levels were higher in the VAP group than in the non-VAP group,and the difference was statistically significant(P<0.05);On days 3 and 5,serum CRP levels were higher in the VAP group than in the non-VAP group,and the difference was statistically significant(P<0.05);On days 1 and 3,serum WBC levels were high⁃er in the VAP group than in the non-VAP group,and the difference was statistically significant(P<0.05);Comparison of CRP levels on day 1 and WBC levels on day 5 between the two groups showed no statistically significant difference(P>0.05);The se⁃rum pro-ADM level on day 1 had the highest sensitivity and specificity,with a cut-off value of 2.53 nmol/L.Conclusion:Pro-ADM has a high diagnostic value for VAP,with high sensitivity and specificity in early diagnosis,and can be used for early diag⁃nosis of VAP.
