摘要
目的分析食管癌患者住院费用的影响因素,为降低住院费用提供参考。方法收集某院2017年1月1日-2021年12月31日1195例食管癌患者住院病案首页资料,采用描述性分析、相关性分析、单因素分析、多元线性逐步回归模型和分位数回归模型进行统计分析。结果食管癌患者平均住院费用26322.35元,年均增长率8%,药品费用与住院费用相关性最强(0.912),但占比呈下降趋势且年均增长率-2.06%;单因素分析结果显示对住院费用的影响因素为住院天数、是否手术、合并症;多元逐步回归结果显示住院天数是影响住院费用的首要因素;分位数回归结果显示住院天数、是否手术、年龄、出院年份和合并症在不同分位数点上对食管癌住院费用产生影响。结论住院日是食管癌患者住院费用的控制重点,医院管理人员应加强住院日管控,提高运行效率,并且通过优先引进和使用集采药品,降低食管癌患者住院经济负担。
Objectives This study aims to analyze the influencing factors of hospitalization expenses in patients with esophageal cancer and provide references for reducing hospitalization expenses.Methods The front-page data of 1,195 cases of esophageal cancer patients in a hospital from January 1,2017 to December 31,2021 were collected.Descriptive analysis,correlation analysis,single factor analysis,multiple linear stepwise regression model,and quantile regression model were used for statistical analysis.Results The average hospitalization cost of esophageal cancer patients was 26322.35 yuan,with an average annual growth rate of 8.00%.Drug expenses had the strongest correlation with hospitalization expenses(0.912),but the proportion was declining,and the average annual growth rate was-2.06%.The results of the univariate analysis showed that the influencing factors on hospitalization expenses were the length of stay in the hospital,operation or not,comorbidities,etc.The results of multiple stepwise regression showed that the length of hospital stay was the primary factor affecting hospitalization costs.The results of quantile regression showed that the length of hospitalization,operation or not,age,year of discharge and comorbidities had an impact on the hospitalization cost of esophageal cancer at different quantile points.Conclusions According to univariate and multivariate analysis,hospitalization day is the key point to control the hospitalization expenses of patients with esophageal cancer.Hospital managers should strengthen the control of hospitalization day,improve the operation efficiency,and reduce the economic burden of hospitalization of patients with esophageal cancer by giving priority to the introduction and use of centralized drugs.
