摘要
目的 探索累及淋巴结小野放疗治疗局部晚期非小细胞肺癌的疗效。方法 选取进行累及淋巴结小野放疗的43例局部晚期非小细胞肺癌患者,分析其失败原因,观察无进展生存时间、总生存时间,以评估累及淋巴结小野放疗的可行性。结果 43例患者中位无进展生存时间为8个月,1年无进展生存率为27.4%,中位总生存时间为22个月,1年生存率为72.2%,2年生存率为35.4%。技术失败病例仅1例,占比2.3%。共29例患者发生了放射性肺炎,发生率为67.4%。结论 在免疫治疗非小细胞肺癌的同时可以考虑累及淋巴结小野放疗,以控制放射性肺损伤。对于肺组织受照射剂量超量,无法完成根治性放疗的患者,可以首先考虑累及淋巴结小野放疗,以减少放射性肺炎的发生。
Objective To investigate the curative effect of small-field radiatiotherapy for involved lymph nodes in treating locally advanced non-small cell lung cancer.Method A total of 43 patients with locally advanced non-small cell lung cancer who received small-field radiotherapy for involved lymph nodes were collected.The reasons for their failure were analyzed,and the progression-free survival and overall survival were observed to evaluate the feasibility of radiotherapy for involved lymph nodes.Result The median progression-free survival of the 43 patients was 8 months,the one-year progression-free survival rate was 27.4%,the median overall survival was 22 months,the one-year survival rate was 72.2%,and the two-year survival rate was 35.4%.There was only one case of technical failure,accounting for 2.3%.Radiation pneumonitis occurred in 29 patients,the incidence rate was 67.4%.Conclusion Small-field radiatiotherapy for involved lymph nodes could be considered in immunotherapy for non-small cell lung cancer to control radiation-induced lung injury.For patients whose lung tissue has received excessive radiation doses and could not complete radical radiotherapy,small-field radiotherapy for involved lymph nodes could be considered primary to reduce the radiation pneumonitis.
