摘要
目的通过信息系统规范头孢菌素类抗菌药物皮试管理,促进抗菌药物的合理使用,为其他药品信息化管理提供参考。方法对厦门大学附属妇女儿童医院HIS系统、合理用药辅助决策系统进行改造,利用信息系统对头孢菌素皮试进行规范,同时与患者既往过敏史匹配,协助医生决策。汇总系统改造前后2021年2-7月和2021年9月至2022年2月头孢菌素使用人数、皮试人数、皮试阳性率等数据;同时通过全国安全用药警戒系统(CHPS)检索头孢菌素过敏相关不良反应数据,对信息系统改造前后的效果进行评价。结果信息系统改造后说明书未要求皮试的头孢菌素皮试率明显低于系统改造前(由95.17%下降至7.69%),差异有统计学意义(P<0.05)。皮试阳性率、过敏相关不良反应发生率系统改造前和系统改造后比较,差异均无统计学意义(P>0.05)。结论通过信息系统可迅速规范头孢菌素类抗菌药物的皮试管理,减少不必要的皮试,减轻医药费用负担,且不增加过敏相关不良反应发生风险。
Objective To standardize the management of cephalosporin antibiotics skin test through the information system,promote the rational use of antibiotics,and provide a reference for the information management of other drugs.Methods The HIS system and the rational drug use decision support system of the Women and Children’s Hospital Affiliated to Xiamen University were modified,and the information system was used to standardize the cephalosporin skin test,with the information matched with the patients’past allergic history to assist doctors in making decisions.The data on the number of cephalosporin users,number of receiving skin tests,and positive rate of skin tests from February to July 2021 and from September 2021 to February 2022 before and after the system modification were summarized.At the same time,adverse reaction data related to cephalosporin allergies were retrieved through the China Hospital Pharmacovigilance System(CHPS)to evaluate the effectiveness of the information system before and after modification.Results The cephalosporin skin test rate,which was not required by the instructions after the information system modification,was significantly lower than that before the system modification(decreased from 95.17%to 7.69%),with statistically significant difference(P<0.05).There were no statistically significant differences in the positive rate of skin test and the incidence of allergic-related adverse reactions before and after the system modification(P>0.05).Conclusion Through the information system,the management of skin tests for cephalosporin antibiotics can be quickly standardized,with reduced unnecessary skin tests,alleviated burden of medical expenses,and no increase in the risk of allergic-related adverse reactions.
