血清S100B联合神经元特异性烯醇化酶检测在高血压脑出血中的临床应用 被引量:1

摘要 目的探讨血清S100钙结合蛋白B(S100B)联合神经元特异性烯醇化酶(NSE)检测在高血压脑出血(HICH)中的应用效果。方法选取武汉科技大学附属天佑医院收治的HICH患者50例作为观察组,根据格拉斯哥昏迷量表(GCS)评分将其分为轻度组(GCS评分13~15分,15例)、中度组(GCS评分9~12分,19例)和重度组(GCS评分3~8分,16例)。另选取同期15例健康者作为对照组。随访3个月,根据患者格拉斯哥预后(GOS)评分将患者分为预后良好组27例(GOS评分4~5分)和预后不良组23例(GOS评分1~3分)。采用酶联免疫吸附法分别测定S100B和NSE水平,Spearman法分析S100B、NSE与预后的相关性。结果各时间段轻度组、中度组、重度组S100B水平与对照组比较,差异有极显著性统计学意义(P<0.01);各时间段中度组、重度组S100B水平与轻度组比较,差异有统计学意义(P<0.05);入院时、入院24 h及入院48 h,中度组S100B水平与重度组比较,差异有统计学意义(P<0.05)。各时间段轻度组、中度组、重度组NSE水平与对照组比较,差异有极显著性统计学意义(P<0.01);入院48 h及72 h重度组NSE水平与轻度组比较,差异有统计学意义(P<0.05)。Spearman相关性分析显示,S100B、NSE水平与HICH患者入院时GCS评分呈负相关(r=-0.876、-0.721,P<0.01),与随访3个月时GOS评分呈负相关(r=-0.421、-0.309,P<0.05)。预后良好组与预后不良组患者S100B、NSE水平比较,差异有统计学意义(P<0.05)。结论S100B联合NSE水平用于评估HICH患者疾病严重程度及判定患者预后的效果较好。 Objective To investigate the application effect of serum S100 calcium binding protein B(S100B)combined with neuron specific enolase(NSE)detection in hypertensive intracerebral hemorrhage(HICH).Methods A total of 50 patients with HICH admitted to Tianyou Hospital Affiliated to Wuhan University of Science and Technology were selected as the observation group.According to Glasgow coma scale(GCS)score,they were divided into the mild group(with GCS score of 13 to 15 points,15 cases),the moderate group(with GCS score of 9 to 12 points,19 cases)and the severe group(with GCS score of 3 to 8 points,16 cases).Another 15 healthy people in the same period were selected as the control group.After 3-month followed-up,according to Glasgow outcome scale(GOS)score,the patients were divided into the good prognosis group(with GOS score of 4 to 5 points,27 cases)and the poor prognosis group(with GOS score of 1 to 3 points,23 cases).The levels of S100B and NSE were measured by enzyme-linked immunosorbent assay,and the correlation of S100B and NSE with prognosis was analyzed by Spearman method.Results The S100B levels in the mild group,the moderate group and the severe group in each time period were significantly different from those in the control group(P<0.01).The S100B levels in the moderate group and the severe group were significantly different from that in the mild group(P<0.05).The S100B levels at admission,24 hours and 48 hours after admission in the moderate group were significantly different from those in the severe group(P<0.05).The NSE levels in the mild group,the moderate group and the severe group in each time period were significantly different from that in the control group(P<0.01).There were statistically significant differences in the NSE levels 48 hours and 72 hours after admission between the severe group and the mild group(P<0.05).Spearman correlation analysis showed that S100B and NSE levels were negatively correlated with the GCS score of HICH patients at admission(r=-0.876,-0.721;P<0.01),and negatively correlated with the GOS score 3 months after follow-up(r=-0.421,-0.309;P<0.05).There were statistically significant differences in the S100B and NSE levels between the good prognosis group and the poor prognosis group(P<0.05).Conclusion S100B combined with NSE level is effective in assessing the severity of disease and determing the prognosis of HICH patients.
