摘要
多囊卵巢综合征(PCOS)是一种妇科内分泌疾病,临床主要表现为稀发排卵或无排卵。目前PCOS的辨治以肾为主,肾主生殖,与PCOS的发生发展密切相关。因此深入开展PCOS肾虚本质的机理研究,有望解决PCOS症状异质性、病机复杂性的难题,为本病治疗提供重要思路。代谢组学可从整体层面反映机体代谢谱的变化规律,从微观视角体现PCOS患者机体的内源性细胞产物和代谢通路的动态性变化过程。本文就PCOS与肾虚病机间的分子代谢内涵、随年龄增长发生的肾虚病机演变,以及基于代谢组学技术探讨从肾论治PCOS的诊疗效果三个方面的研究进展进行综述。发现PCOS肾虚内在机理与氨基酸、嘌呤、柠檬酸、碳水化合物及脂类等多种代谢途径的异常有关,通过干预氧化应激、能量代谢等过程,进而改变卵泡的供能方式与线粒体活性,影响了机体状态、卵泡发育与妊娠的结局,增加远期并发肿瘤与心血管疾病的风险。
Polycystic ovary syndrome(PCOS)is a gynecological endocrine disease characterized by sparse ovulation or anovulation.Modern doctors believe that the diagnosis and treatment of PCOS is mainly based on Kidney,which governs reproduction and is closely related to the occurrence and development of PCOS.Therefore,conducting in-depth research on the mechanism of Kidney-deficiency in PCOS is hopeful to solve the problem of heterogeneous symptoms and complex pathogenesis of PCOS,and provide important ideas for the treatment of this disease.Metabolomics can reflect the changes in the body's metabolic spectrum from a holistic perspective,and reflect the dynamic changes in endogenous cellular products and metabolic pathways in PCOS patients from a microscopic perspective.This study reviewed the molecular metabolic connotation between PCOS and the pathogenesis of Kidney-deficiency,the evolution of Kidney-deficiency pathogenesis with age,and the research progress based on metabolomics technology to explore the diagnostic and therapeutic effects of treating PCOS from the perspective of Kidney.It has been found that the intrinsic Kidney-deficiency mechanism of PCOS is related to abnormalities in various metabolic pathways including pathways of amino acids,purines,citric acid,carbohydrates and lipids.By intervening in processes such as oxidative stress and energy metabolism,the energy supply mode of follicles and mitochondrial activity are altered,affecting the body's state,follicular development and pregnancy outcomes,increasing the risk of long-term complications of tumors and cardiovascular diseases.
