艾滋病相关机会性感染病原菌分布及耐药性分析

摘要 目的探讨艾滋病相关机会性感染病原菌的分布特点并分析其耐药性。方法此研究选取天津市第二人民医院收治的艾滋病患者为研究对象,观察患者2019年1月-2020年12月期间多重耐药菌分布情况、观察并探讨患者入住ICU时间与多重耐药菌感染情况的相关性,探讨该研究选取标本的来源情况、耐药情况。结果共培养出阳性结果3264株,其中多重耐药菌1254株,研究中共分离大肠埃希菌276株,对头孢吡的耐药率为66.30%,对亚胺培南的耐药率为4.71%,对哌拉西林的耐药率为17.39%;共分离铜绿假单胞菌142株,对头孢吡的耐药率为64.79%,对亚胺培南的耐药率为63.38%,对哌拉西林的耐药率为52.82%;共分肺炎克雷伯菌231株,对头孢吡的耐药率为49.78%,对亚胺培南的耐药率为20.35%,对哌拉西林的耐药率为9.52%;共分离金黄色葡萄球菌23株,对苯唑西林的耐药率为73.91%,对红霉素的耐药率为65.22%,对利奈唑胺的耐药率为4.35%。结论艾滋病患者感染病原菌分布较广,耐药率较高,为避免长期应用广谱抗菌药物而造成细菌耐药,需加强对艾滋病机会性感染病原菌耐药性的动态监测。 Objective To investigate the distribution and drug resistance of HIV-associated Opportunistic infection pathogens in patients with AIDS.Method To observe the distribution of multi-drug resistant bacteria in patients from January 2019 to December 2020,and to explore the correlation between the time of ICU stay and multi-drug resistant bacteria infection,to investigate the source and drug resistance of the selected samples.Results A total of 3264 strains were positive,of which 1254 strains were multi-drug resistant.276 strains of Escherichia coli were isolated.The resistance rate to cefpixime and imipenem was 66.30% and 4.71%respectively,the resistant rate to peracillin was 17.39%.142 strains of Pseudomonas aeruginosa were isolated.The resistant rate to cefepime,imipenem and peracillin were 64.79%,63.38%and 52.82%,respectively,a total of 231 strains of klebsiella pneumoniae were isolated.The resistant rates to cefepime,imipenem and peracillin were 49.78%,20.35%and 9.52%,respectively,the drug resistance rates to oxacillin,erythromycin and Linezolid were 73.91%,65.22%and 4.35%.Conclusions Respectively,high drug resistance to avoid the long-term use of broad-spectrum antibiotics caused by bacterial resistance,it is necessary to strengthen the dynamic monitoring of drug resistance of Opportunistic infection pathogens of AIDS.
