摘要
目的:观察改良式眼轮匝肌折叠术治疗老年复发性下睑内翻的美学效果及对并发症的影响。方法:选取2018年6月-2021年6月北京市顺义区医院收治的80例(94眼)老年下睑内翻患者作为研究对象,根据非随机临床同期对照研究及患者自愿原则分组,43例(51眼)实施改良式眼轮匝肌折叠术,纳入观察组;37例(43眼)实施常规眼睑皮肤轮匝肌切除术,纳入对照组。比较两组近期疗效,术前、术后6个月眼部症状(异物感、刺痛、畏光)评分、眼部形态美学指标(睑裂倾斜度),术后并发症发生情况、近远期复发情况及美容满意度。结果:观察组手术总有效率高于对照组(P<0.05);两组术后6个月眼部异物感、刺痛、畏光症评分均较术前降低(P<0.05),且观察组低于对照组(P<0.05);两组术后6个月睑裂倾斜度均较术前增大(P<0.05),且观察组大于对照组(P<0.05);观察组术后并发症总发生率及术后1年复发率小于对照组(P<0.05);观察组术后满意度高于对照组(P<0.05)。结论:改良式眼轮匝肌折叠术治疗老年复发性下睑内翻疗效确切,并发症少,远期复发率低,且美学效果较好。
Objective To observe the cosmetic effect of modified orbicularis muscle folding in the treatment of elderly recurrent lower eyelid entropion and its influence on complications.Methods Eighty elderly patients(94 eyes)with lower entropion treated in Shunyi district hospital from June 2018 to June 2021 were selected as the study objects,and they were grouped according to the non-randomized clinical concurrent control study and the principle of voluntariness.43 cases(51 eyes)carried out modified orbicularis muscle folding and were included in the observation group,and 37 cases(41 eyes)underwent routine eyelid skin orbicularis muscle resection and were enrolled as the control group.The short-term efficacy,scores of ocular symptoms(foreign body sensation,tingling pain,photophobia)and ocular morphological and aesthetic indicators(palpebral fissure inclination)before surgery and at 6 months after surgery,occurrence of postoperative complications,short-term and long-term recurrence and cosmetic satisfaction were compared between the two groups.Results The total effective rate of surgery in the observation group was higher than that in the control group(P<0.05).The scores of foreign body sensation,stabbing pain and photophobia in the two groups at 6 months after surgery were all lower than those before surgery(P<0.05),and the scores in the observation group were lower than the control group(P<0.05).The inclination of palpebral fissure in the two groups was enlarged at 6 months after surgery compared to before surgery(P<0.05),and the inclination was larger in the observation group than the control group(P<0.05).The total incidence rate of postoperative complications and 1-year recurrence rate in the observation group were lower than those in the control group(P<0.05)while the postoperative satisfaction was higher than that in the control group(P<0.05).Conclusion Modified orbicularis muscle folding is effective in the treatment of elderly recurrent lower eyelid entropion,with few complications,low long-term recurrence rate and good cosmetic effect.
