摘要
目的分析呼吸道感染患儿生化指标、凝血功能指标和免疫功能相关指标的变化,并探究其临床意义。方法抽取2019年1月至2019年12月郑州大学第三附属医院收治的呼吸道感染患儿1011例,并按照疾病类型分为上呼吸道感染组、支气管炎组、毛细支气管炎组及肺炎组,同时抽取同期健康体检儿童318例作为对照组。搜集所有研究对象的一般临床资料,并采集所有研究对象的晨起空腹静脉血,离心后检测生化指标、凝血功能及免疫功能相关指标。结果五组纳入研究对象中,毛细支气管炎组患儿年龄最小,为0.58(0.37,0.92)岁。与对照组相比,上呼吸道感染组、支气管炎组、毛细支气管炎组、肺炎组患儿的丙氨酸氨基转移酶、天门冬氨酸氨基转移酶、γ-谷氨酰转移酶表达轻度升高,尿素、肌酐、尿酸、血清总胆红素、总蛋白及白蛋白水平均有不同程度的降低(P<0.05)。与对照组相比,上呼吸道感染组、支气管炎组、毛细支气管炎组、肺炎组患儿的凝血酶原时间、活化部分凝血活酶时间均延长,纤维蛋白原、血浆纤维蛋白原降解产物、血浆D二聚体水平均升高(P均<0.05)。与对照组相比,上呼吸道感染组、支气管炎组、毛细支气管炎组、肺炎组患儿的IgG、IgA水平均降低,尤其是毛细支气管炎组,明显低于对照组和其他呼吸道感染组,而补体C3、C4均有不同程度的增高(P均<0.05)。结论呼吸道感染患儿的生化、凝血及体液免疫功能指标存在异常,在疾病的诊断和治疗中具有一定的临床价值。
ObjectiveTo analyze the changes in biochemical indexes,coagulation function indexes,and immune function related indexes in children with respiratory tract infections,and to investigate the clinical significance.MethodsA total of 1011 children with respiratory tract infections admitted to the Third Affiliated Hospital of Zhengzhou University from January 2019 to December 2019 were selected,and they wee divided into upper respiratory tract infection group,bronchitis group,bronchiolitis group and pneumonia group according to disease type.At the same time,318 children who underwent physical examinations during the same period were selected as control group.The general clinical data of all study subjects were collected;fasting venous blood was collected from all selected patients in the morning,and were tested for biochemical indexes,coagulation function,and immune function after centrifugation.ResultsAmong the five groups included in the study,the bronchiolitis group was the youngest,aged 0.58(0.37,0.92)years.Compared with the control group,the expression of alanine aminotransferase,aspartate aminotransferase,andγ-glutamyltransferase in the upper respiratory tract infection group,bronchitis group,bronchiolitis group and pneumonia group increased slightly,while levels of urea,creatinine,uric acid,serum total bilirubin,total protein,and albumin all decreased to varying degrees(P<0.05).Compared with the control group,the prothrombin time and activated partial thromboplastin time in the upper respiratory tract infection group,bronchitis group,bronchiolitis group,pneumonia group all prolonged,moreover,the levels of fibrinogen,plasma fibrinogen degradation products and plasma D-dimer increased(all P<0.05).The levels of immunoglobulin G and immunoglobulin A in the upper respiratory tract infection group,bronchitis group,bronchiolitis group and pneumonia group were lower than those in the control group,especially,the levels of immunoglobulin G and immunoglobulin A in the bronchiolitis group were significantly lower than those in the control group and other respiratory tract infection groups;while levels of complement 3 and complement 4 in the four respiratory tract infection groups were higher than those in the control group(all P<0.05).ConclusionsThe biochemical indexes,coagulation function indexes and immune function indexes are abnormal in children with respiratory tract infections,and they have clinical value in the diagnosis and treatment of the diseases.
