摘要
目的探讨原发性高血压患者肾功能生物学标志物及其他临床指标对其脑小血管病(CSVD)发生的影响。方法选取2018年1月-2020年6月贵州医科大学附属医院高血压科286例住院的原发性高血压患者,根据头颅核磁共振检查结果分为CSVD组(178例),NCSVD组(108例)。比较两组血肌酐、尿酸、胱抑素C和尿微量蛋白/肌酐检测,并估测肾小球滤过率(e'GFR),对有差异指标行多因素Logistic回归分析,找出CSVD的独立危险因素。结果两组尿微量蛋白/肌酐、血胱抑素C、e'GFR、年龄、体重、高血压病程、空腹血糖、血同型半胱氨酸水平比较,差异有统计学意义(P<0.05);Logistic回归分析显示,CSVD的独立危险因素包括年龄、空腹血糖(OR=1.112、1.337,P<0.05)和体重,而体重是CSVD的保护因素(OR=0.959,P<0.05)。结论尿微量蛋白/肌酐、胱抑素C、e'GFR可作为原发性高血压患者预测CSVD的重点监测指标,而年龄越大、空腹血糖越高、体重越低,原发性高血压患者CSVD患病风险越高。
Objective To investigate the effect of biological markers of renal function and other clinical indicatorson on cerebral small-vessel disease(CSVD)in patients with primary hypertension.Methods A total of 286 patients with essential hypertension who were hospitalized in the Department of Hypertension,Affiliated Hospital of Guizhou Medical University from January 2018 to June 2020 were selected and divided into CSVD group(178 patients)and NCSVD group(108 patients)according to the results of cranial magnetic resonance imaging.Serum creatinine,uric acid,cystatin C and urinary microprotein/creatinine were compared between the two groups,and glomerular filtration rate(e'GFR)was estimated.Multivariate logistic regression analysis was performed on the different indicators to find out the independent risk factors of CSVD.Results There were significant differences in urinary microprotein/creatinine,serum cystatin C,e'GFR,age,weight,duration of hypertension,fasting blood glucose and blood homocysteine levels between the two groups(P<0.05).Logistic regression analysis showed that age,fasting blood glucose(OR=1.112,1.337,P<0.05)and body weight were independent risk factors for CSVD,while body weight was a protective factor for CSVD(OR=0.959,P<0.05).Conclusion Urinary microprotein/creatinine,cystatin C,and e'GFR can be used as key monitoring indicators for predicting CSVD in patients with essential hypertension.The older the age,the higher the fasting blood glucose,and the lower the weight,the higher the risk of CSVD in patients with essential hypertension.
