摘要
目的:分析股骨近端防旋髓内钉(PFNA)与InterTan髓内钉内固定治疗老年股骨转子间骨折的临床疗效。方法:选取2021年1—2022年12月清镇市中医医院收治的老年股骨转子间骨折患者38例为研究对象,其中InterTan髓内钉内固定23例(InterTan组),PFNA内固定15例(PFNA组)。比较两组患者临床指标、并发症发生情况。结果:PFNA组手术时间短于InterTan组,PFNA组术中失血量少于InterTan组,InterTan组骨折愈合时间短于PFNA组(P<0.05);两组患者并发症发生率比较,差异无统计学意义(P>0.05)。结论:PFNA内固定与InterTan内固定治疗老年股骨转子间骨折,早期疗效均显著。但InterTan内固定治疗具有更加优良的抗旋转稳定性与轴向加压作用,内固定有效率更高,与PFNA内固定比较,其并发症发生率稍低。InterTan内固定可以帮助患者实现更早期的下床活动,缩短卧床时间,减少术后下肢深静脉血栓。
Objective:To analyze the clinical efficacy of proximal femoral anti-rotation intramedullary nail(PFNA)and InterTan intramedullary nail fixation in the treatment of elderly intertrochanteric fracture of femur.Methods:Thirty-eight elderly patients with intertrochanteric fracture of femur treated in Qingzhen Hospital of Traditional Chinese Medicine from January 2021 to December 2022 were selected as the study objects,including twenty-three patients with InterTan intramedullary nail fixation(InterTan group)and fifteen patients with PFNA fixation(PFNA group).The clinical indexes and complications of the two groups were compared.Results:The operative time in PFNA group was shorter than that in InterTan group,the intraoperative blood loss in PFNA group was shorter than that in InterTan group,and the fracture healing time in InterTan group was shorter than that in PFNA group(P<0.05);There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:PFNA internal fixation and InterTan internal fixation in the treatment of senile intertrochanteric fractures have significant early curative effects.However,compared with PFNA internal fixation,InterTan internal fixation has better anti-rotation stability and axial compression effect,higher internal fixation efficiency,and slightly lower complication rate.InterTan internal fixation can help patients get out of bed earlier,shorten bed time,and reduce postoperative deep venous thrombosis of lower limbs.
