摘要
目的:探讨径线比值(S/L)在良、恶性肺结节鉴别诊断中的价值,为影像学鉴别诊断提供一种新思路。方法:选取2018年3月—2022年2月期间于山东大学齐鲁医院德州医院经胸部CT检查发现并经过病理证实的肺结节患者共57例(恶性组35例,良性组22例)为研究对象,经CT扫描后三维重建,选取病灶最大层面,沿病灶中心三个相互垂直的最大径线测量,取差值最大的长径(L)与短径(S)两个值,计算S/L值。结果:恶性组和良性组的S/L值分别为(0.76±0.11)(0.57±0.17),差异有统计学意义(t=6.753,P<0.05);良恶性结节S/L值的ROC曲线下面积为0.86,最佳诊断界值为0.64,当S/L值大于0.64时,其诊断肺良恶性结节的灵敏度为82.9%,特异度为72.7%。结论:径线比(S/L)值可以定量评估肺结节的形态特征,在良恶性肺结节鉴别诊断中具有较高价值。
Objective To investigate the value of diameter/line ratio(S/L)in the differential diagnosis of benign and malignant pulmonary nodules,and to provide a new idea for imaging differential diagnosis.Methods A total of 57 patients with pulmonary nodules(35 in the malignant group and 22 in the benign group)who underwent chest CT examination and pathological confirmation at Shandong University Qilu Hospital Dezhou Hospital from March 2018 to February 2022 were selected as the study subjects.After CT scanning,three-dimensional reconstruction was performed,the maximum layer of the lesion was selected and measured along the three perpendicular maximum diameters of the center of the lesion.The S/L was calculated by taking the two values of the major diameter(L)and the minor diameter(S)with the largest difference.Results The S/L of malignant group and 22 benign group were(0.76±0.11)and(0.57±0.17),respectively,the difference was statistically significant(t=6.753,P<0.05).The area under the ROC curve of S/L of benign and malignant pulmonary nodules was 0.86,and the best diagnostic cut-off value was 0.64.When the S/L was greater than 0.64,the sensitivity and specificity of diagnosing benign and malignant pulmonary nodules were 82.9%and 72.7%,respectively.Conclusion The diameter/line ratio(S/L)can quantitatively evaluate the morphological characteristics of pulmonary nodules,and has a high value in the differential diagnosis of benign and malignant pulmonary nodules.
