摘要
冠状动脉血运重建术存在破坏微血管结构和功能,并诱发心绞痛的风险。根据脉络学说,以及孙络与微血管的相关性,冠状动脉血运重建术后心绞痛的中医病机为素体虚弱和术中破血,术后抗凝导致络气虚滞,术中及术后紧张焦虑导致络气郁滞,痰瘀阻于脉络导致营卫不通,以及营卫失调导致脉络绌急。治疗方面,络气虚滞者以益气活血、调和营卫为主;络气郁滞者重在理气活血,以畅达脉络;痰瘀阻络者应治以活血化瘀、祛痰宣痹之法;脉络绌急者,辅以虫类药,以其搜风解痉之用,缓解脉络绌急痉挛的病理状态。
Coronary revascularization has the risk of disrupting the structure and function of the microvasculature and inducing angina pectoris.According to the chakra theory and the correlation between Sunluo and microvasculature,the pathogenesis of angina pectoris after coronary artery revascularization surgery in traditional Chinese medicine is weakness of the body and intraoperative rupture of blood.Postoperative anticoagulation leads to deficiency and stagnation of luo Qi,tension and anxiety during and after surgery leads to stagnation of luo Qi,phlegm and stasis obstructing the venation leads to obstruction of ying-wei,and dysfunction of ying-wei leads to urgency of the venation.In terms of treatment,patients with deficiency and stagnation of luo Qi mainly focus on tonifying Qi,promoting blood circulation,and harmonizing yin and wei.For those with stagnation of luo Qi,the focus is on regulating Qi and promoting blood circulation to facilitate smooth flow of the veins.For those with phlegm and stagnation of luo,it should be treated with the methods of promoting blood circulation and resolving blood stasis,dispelling phlegm and promoting obstruction.For those with dense vasculature,the treatment is supplemented with insect drugs,which is used to search the wind and relieve spasm,so as to relieve the pathological state of dense vasculature.
