摘要
目的在常规肋骨三维CT重建基础上,探讨采用术中彩超,对肋骨骨折断端精准定位的效果。方法回顾性分析2019年1月~2022年1月行肋骨骨折内固定术(SSRF)120例患者的临床资料。分为单纯行肋骨三维CT重建组(50例),肋骨三维CT重建+术中彩超定位组(70例),比较两组切口的长度、手术时间、胸管引流量及拔管时间、术后第1天疼痛评分(VAS)及住院时间。结果三维CT+术中超声定位组在手术切口的长度、手术时间、术中出血量、术后第1天VAS评分等方面均占优势,但在胸管引流量、术后拔管时间及住院天数方面差异无统计学差异。结论应用彩超定位可对肋骨骨折断端精准定位,指导手术切口的选择,减轻手术创伤,利于患者快速康复。
Objective To investigate the effect of intraoperative color ultrasonography on the accurate location of broken end of rib fracture on the basis of conventional three-dimensional CT(3D-CT).Methods The data of 120 patients who underwent surgical stabilization of rib fractures(SSRF)from January 2019 to January 2022 were retrospectively analyzed.The patients were divided into the rib 3D CT reconstruction group(50 cases)and the rib 3D CT reconstruction+intraoperative color ultrasound localization group(70 cases).The length of incision,operative time,thoracic duct drainage flow rate and extubation time,postoperative pain score(VAS)and hospitalization time were compared between the two groups.Results The 3D-CT+intraoperative ultrasound localization group was superior in the length of the surgical incision,the operative time,the intraoperative blood loss,and the VAS score on the first day after the operation.But there was no statistical difference in the thoracic duct drainage flow rate,the postoperative extubation time,and the hospitalization time.Conclusion The application of color ultrasound positioning during SSRF can accurately locate the broken ends of rib fractures,guide the selection of surgical incisions,reduce surgical trauma,and facilitate the rapid recovery of patients.
