摘要
目的 研究切开复位内固定、微创经皮钢板接骨术(minimally invasive percutaneous piate osteosyndesis,MIPPO)结合锁定加压钢板(locking compression plate, LCP)内固定治疗胫骨远端闭合性骨折的临床价值。方法选取2020年4月—2021年12月深圳市龙华区中心医院创伤骨科收治的胫骨远端闭合性骨折患者60例。按照随机抽样分组法分为两组,参比组30例患者应用切开复位内固定治疗,观察组30例患者应用MIPPO技术结合LCP钢板内固定治疗。比较两组治疗后的疼痛情况、踝关节功能评分、膝关节功能评分、临床愈合时间、临床疗效、并发症发生情况。结果 治疗后,观察组患者的疼痛情况轻于参比组,差异有统计学意义(P<0.05);观察组患者的踝关节功能评分、膝关节功能评分、临床愈合时间均优于参比组,差异有统计学意义(P<0.05);观察组患者的临床疗效高于参比组,差异有统计学意义(P<0.05);观察组患者的并发症发生率为3.33%,低于参比组的26.67%,差异有统计学意义(χ^(2)=4.706,P<0.05)。结论 在对胫骨远端闭合性骨折患者进行治疗时,使用MIPPO技术结合LCP钢板内固定治疗能够促使患者术后疼痛情况消失,改善患者膝关节功能,加快患者骨折愈合,提高临床疗效,减少并发症情况发生,值得使用。
Objective To investigate the clinical value of open reduction internal fixation,minimally invasive percuta⁃neous piate osteosyndesis(MIPPO)combined with locking compression plate(LCP)internal fixation in the treatment of closed fracture of distal tibia.Methods A total of 60 patients with closed fracture of the distal tibia admitted to the Trauma and Orthopedics Department of Longhua District Central Hospital of Shenzhen from April 2020 to December 2021 were selected.According to random sampling,the patients were divided into two groups.The reference group(30 cases)received open reduction and internal fixation treatment,and the observation group(30 cases)received MIPPO technology combined with LCP plate internal fixation treatment.The pain,ankle joint function score,knee joint func⁃tion score,clinical healing time,clinical efficacy and complications were compared between the two groups after treat⁃ment.Results After treatment,the pain in observation group was milder than that in reference group,the difference was statistically significant(P<0.05).The ankle joint function score,knee joint function score and clinical healing time of observation group were better than those of reference group,the difference was statistically significant(P<0.05).The clinical efficacy of observation group was higher than that of reference group,the difference was statistically significant(P<0.05).The complication rate of the observation group was 3.33%,which was lower than that of the refer⁃ence group (26.67%), the difference was statistically significant (χ2=4.706, P<0.05). Conclusion In the treatment of patients with closed fractures of the distal tibia, MIPPO technique combined with LCP plate internal fixation can make the postoperative pain disappear, improve the function of the knee joint, accelerate the healing of the fracture, improve the clinical efficacy, and reduce the occurrence of complications, which is worthy of use.
