摘要
目的探究骨质疏松性椎体骨折经皮球囊扩张椎体后凸成形术(pereutaneous kyphopl asty,PKP)后残留疼痛程度的影响因素。方法回顾性分析2021年7月—2022年12月张家港澳洋医院骨科符合条件的60例骨质疏松性椎体骨折PKP术后患者的资料,按照术后是否存在残留疼痛分为对照组[视觉模拟评分法(Visual Analogue Scale,VAS)评分<3分]和观察组(VAS评分≥3分),每组30例。评估两组患者术后存在椎体残余疼痛单因素,并进行多因素分析。结果两组性别、体质指数(Body Mass Index,BMI)、骨水泥注入量、骨水泥渗漏及术前椎体前缘高度比较,差异无统计学意义(P>0.05);两组病因、病史、骨密度(bone mineral density,BMD)、椎体前缘高度恢复情况、椎体中间高度恢复情况比较,差异有统计学意义(P<0.05);病因、病史、BMD、椎体前缘高度恢复情况、椎体中间高度恢复情况是残余疼痛程度的高危因素(OR=1.770、1.330、1.170、1.340、1.860,P<0.05)。结论骨质疏松性椎体骨折PKP术后残留疼痛程度的影响因素较多,故后续治疗过程中应注意其影响因素,以期可以缓解患者的术后残留疼痛程度,大大降低术后残留疼痛的发生率。
Objective To explore the influencing factors of residual pain after percutaneous balloon kyphoplasty(PKP)for osteoporotic vertebral fractures.Methods The data of 60 patients with osteoporotic vertebral fracture after PKP in the Department of Orthopedics,Zhangjiagang Aoyang Hospital from July 2021 to December 2022 were retrospectively analyzed.According to whether there was residual pain after operation,they were divided into control group[Visual Analogue Scale(VAS)score<3 points]and observation group(VAS score≥3 points),with 30 cases in each group.The single factor of postoperative residual vertebral pain in the two groups was evaluated and multivariate analysis was performed.Results There was no statistically significant difference in gender,body mass index(BMI),bone cement injection volume,bone cement leakage and preoperative anterior vertebral height between the two groups(P>0.05).There were statistically significant differences in etiology,medical history,bone mineral density(BMD),recovery of anterior vertebral height and recovery of middle vertebral height between the two groups(P<0.05).Etiology,medical history,BMD,recovery of anterior vertebral height and recovery of middle vertebral height were high risk factors for residual pain(OR=1.770,1.330,1.170,1.340,1.860,P<0.05).Conclusion There are many factors that affect the degree of residual pain after PKP surgery for osteoporotic vertebral fractures,so attention should be paid to the influencing factors during subsequent treatment.It is hoped that it can alleviate the patient's residual pain after surgery and greatly reduce the incidence of residual pain after surgery.
