摘要
目的探讨全身振动疗法对痉挛型脑瘫患儿坐位平衡功能状态的影响。方法选取2021年11月至2022年5月痉挛型脑瘫患儿30例,采用随机数字表法分为2组,各15例。对照组患儿接受常规的综合康复训练,观察组在对照组基础上辅以全身振动疗法,干预时间总计3个月,5次/周,15 min/次。治疗前后两组患儿的粗大功能评定量表GMFM-88项D区(站立位)和E区(行走、跑、跳)评分,儿童平衡量表总得分(PBS),5次坐立试验(FTSST),10 m步行时间(10 MWT),竖脊肌肌电值(aEMG)等指标进行评定。结果两组患儿治疗后GMFM D区和E区评分均有所增高,且观察组治疗后GMFM D区[(39.93±4.33)分]和E区[(64.53±12.81)分]评分均高于对照组;两组治疗后FTSST和10 MWT有所下降,观察组PBS评分有所提高,且观察组治疗后FTSST[(9.31±3.41)s]和10 MWT[(0.95±0.12)s]均低于对照组,PBS评分[(51.40±3.74)分]高于对照组;两组患儿治疗后左右侧竖脊肌aEMG均有所提高,且观察组治疗后竖脊肌aEMG[左侧(68.13±13.89)μV,右侧(65.95±12.33)μV]均高于对照组。结论对于痉挛型脑瘫患儿在常规康复训练基础上结合全身振动疗法能够有效改善肌肉的痉挛状态,提高核心肌群及下肢肌力,增强核心稳定性,提高患儿的坐位及整体运动平衡能力。
Objective To explore the effects of whole body vibration therapy on the seat balance function of children with spastic cerebral palsy.Methods A total of 30 children with spastic cerebral palsy who were treated in the outpatient rehabilitation department of the Children's Rehabilitation Department of Yiwu Maternal and Child Health Hospital from November 2021 to May 2022 were selected,and the subjects were divided into two groups by random number table method,namely the control group and the observation group,15 cases each.The patients in the control group received conventional comprehensive rehabilitation training,while the observation group received whole-body vibration therapy on the same basis.The intervention time was 3 months,5 times a week,15 minutes each time.Before and after treatment,the gross functional rating scale GMFM-88 item D area(standing)and E area(walking,running,jumping)scores of the two groups of children,the total score of children's balance scale(PBS),5 sitting tests(FTSST),10 m walking time(10 MWT),and the average electromyographic(aEMG)of the erector spinae were evaluated.Results The two groups of children showed that the scores of GMFM D area(standing position)and E area(walking,running,jumping)increased after treatment,and the observation group after treatment GMFM D area[(39.93±4.33)points]and E area[(64.53±12.81)points]scores were higher than those in the control group;the FTSST and 10 MWT of the two groups decreased after treatment,and the PBS score of the observation group increased,and the FTSST[(9.31±3.41)seconds]and 10 MWT[(0.95±0.12)seconds]of the observation group were lower than those of the control group,and the PBS score[(51.40±3.74)points]was higher than that of the control group;after treatment,the aEMG of the left and right erector spinae in the children increased,and the aEMG of the erector spinae in the observation group after treatment[left(68.13±13.89)μV,right(65.95±12.33)μV]were higher than those in the control group.Conclusion For children with spastic cerebral palsy,on the basis of routine rehabilitation training,whole body vibration therapy can effectively improve muscle spasticity,improve core muscle group and lower limb muscle strength,enhance core stability,and improve children's sitting and overall movement balance ability.
