摘要
目的探讨前列腺癌(PCa)与前列腺增生(BPH)鉴别中MRI动态增强扫描扩散加权成像(DWI)以及表观扩散系数(ADC)值的价值。方法选取2021年5月至2022年12月宁波市医疗中心李惠利医院收治进行前列腺MRI检查的80例患者,均经过病理检查确诊,其中PCa患者40例,BPH患者40例。所有患者均行MRI常规扫描以及动态增强扫描,自动生成DWI以及ADC图像,再选取感兴趣区域(ROI)绘制时间-信号强度曲线图,得出影像学诊断结果后与病理检查结果进行对比分析。结果时间-信号强度曲线分型中BPH患者以Ⅱ型为主,PCa患者以Ⅰ型为主,差异有统计学意义(P<0.05)。DWI中PCa以高信号为主,BPH以等低信号为主;ADC信号中PCa以等低信号为主,BPH以高信号为主,差异有统计学意义(P<0.05)。PCa组平均ADC值为(0.93±0.21)×10^(-3)mm^(2)/s,BPH组为(1.62±0.16)×10^(-3)mm^(2)/s,两组间比较差异有统计学意义(P<0.01)。ROC曲线显示,DWI和ADC值鉴别诊断PCa和BPH的AUC为0.894,灵敏度为0.825,特异度为0.875。结论PCa以及BPH在MRI动态增强扫描中DWI以及ADC表现为不同信号,对两者鉴别诊断具有一定的价值。
Objective To evaluate the application of diffusion weighted imaging(DWI)and apparent diffusion coefficient(ADC)value of dynamic contrast-enhanced MRI in the differentiation of prostate cancer(PCa)and benign prostate hyperplasia(BPH).Methods Eighty patients with pathologically confirmed PCa(40 cases)and BPH(40 cases)admitted in of Ningbo Medical Center Li Huili Hospital from May 2021 to December 2022 were enrolled in the study.All patients underwent routine MRI and dynamic enhanced MRI scanning and the DWI and ADC were automatically generated,the time signal intensity curves of selecting region of interest(ROI)were drawn.The value of DWI and ADC in differentiation between PCa and BPH were evaluated using pathological diagnosis as the gold standard.Results In the time signal intensity curve classification,BPH patients mainly presented as type II,while PCa patients mainly presented as type I(P<0.05).In DWI,PCa mainly had high signal,while BPH mainly had low signal;in ADC signals,PCa mainly had low signal,while BPH mainly had high signal,with a statistically significant difference(P<0.05).The average ADC value of PCa patients was(0.93±0.21)×10^(-3)mm^(2)/s,while that of BPH was(1.62±0.16)×10^(-3)mm^(2)/s(P<0.01).The AUC of imaging diagnosis was 0.894,the diagnostic sensitivity and specificity was 0.825 and 0.875,respectively.Conclusion PCa and BPH exhibit different signals in DWI and ADC on dynamic contrast-enhanced MRI,which has certain value in differential diagnosis between the two lesions.
