摘要
【目的】在复杂网络技术的基础上探究针灸治疗中风后呃逆的腧穴配伍规律。【方法】计算机检索中国知网期刊全文数据库(CNKI)、万方学术期刊全文数据库(Wanfang)、维普中文科技期刊数据库(VIP)、中国生物医学数据库(CBM)、美国生物医学信息检索系统(PubMed)、荷兰医学文摘(Embase)、国际循证医学图书馆(Cochrane Library)、科学网(web of science)等各大数据库中以针灸为主治疗中风后呃逆的文献。运用SPSS Modeler 18.0、Cytoscape 3.9.1、Gephi 0.9.7软件进行关联规则、网络可视化呈现及复杂网络分析。【结果】最终纳入265篇文献,涉及391条有效处方,共计103个腧穴,总应用频次为1597次;治疗中风后呃逆最主要的治疗措施是毫针针刺,经脉以胃经、任脉、膀胱经和心包经为主;主穴处方中的高频腧穴为足三里,“足三里-内关”为相关性最高的腧穴组合,其次为“足三里-中脘”。经复杂网络“社团分析”可得到3个核心腧穴群。【结论】目前临床研究之间存在较大差异,没有公认的针灸治疗中风后呃逆的腧穴处方及操作规范,该研究通过复杂网络技术进一步总结出针灸治疗中风后呃逆的核心腧穴处方、腧穴归经、配伍原则、刺灸法应用特点等规律,可为临床中针灸治疗本病提供参考依据。
Objective To investigate the compatibility rules of acupoints for acupuncture and moxibustion in the treatment of post-stroke hiccup on the basis of complex network technology.Methods The literature on acupuncture-moxibustion based treatment of post-stroke hiccup were screened in major databases such as the CNKI,Wanfang,VIP,CBM,PubMed,Embase,Cochrane Library,and web of science.SPSS Modeler18.0,Cytoscape 3.9.1 and Gephi 0.9.7 software were used for association rules,network visualization presentation and complex network analysis.Results A total of 265 articles were included,involving 391 valid prescriptions for totally 103 acupoints,with a total application frequency of 1597 times;the predominant therapeutic measure for the treatment of post-stroke hiccup was filiform needle,with the meridians of the stomach,conception vessel(CV1),bladder and pericardium meridians predominating;the high-frequency acupoint in the main acupuncture prescription is the Zusanli(ST36),with the highest correlation being the combination of“Zusanli(ST36)-Neiguan(PC6)”,followed by“Zusanli-Zhongwan(CV12)”.Three core acupoint clusters can be obtained by“association analysis”of complex networks.Conclusion The present study further summarizes the core acupoint prescriptions,acupoint meridian tropism,principles of compatibility,application characteristics of acupuncture and moxibustion methods and other rules of acupuncture and moxibustion for the treatment of post-stroke hiccup through complex network technology,which can provide a reference basis for acupuncture and moxibustion in the clinical treatment of this disease.
