摘要
目的 探讨腔内心电技术(IC-ECG)联合呼吸配合法在肿瘤患者PICC置管过程中的应用效果。方法 按组间基本资料具有可比性的原则将2021年1-10月在科室住院治疗需置入PICC的肿瘤患者255例分为三组,每组85例。分别为常规组(A组)、IC-ECG联合呼吸法组(B组)和IC-ECG联合转头下颌压肩法(C组)。比较导管尖端的到位率、异位率、患者舒适度和操作者置管肯定感。结果 三组患者的到位率和异位率比较差异均有统计学意义(P<0.05),其中B组的到位率最高,异位率最低;而A组的到位率最低,异位率最高。三组操作者肯定感率比较,差异具有统计学意义(P<0.05),其中C组肯定感率最高,A组最低。三组患者舒适度比较差异有统计学意义(P<0.05);其中B组的舒适程度最高,C组最低。结论 IC-ECG联合呼吸配合法在肿瘤患者PICC置管过程中的应用可提高PICC导管尖端的到位率、降低置管过程中导管异位发生率、提高置管患者舒适度以及置管操作者的置管肯定感。
Objective To explore the application effect of intracavitary electrical technique(IC-ECG)combined with respiratory coordination method in the process of PICC placement in oncology patients.Methods According to the principle of comparability of basic information between groups,255 cases of oncology patients who were hospitalised in the department from January to October 2021 and needed to be placed in PICC were divided into three groups,with 85 cases in each group.They were the conventional group(Group A),the IC-ECG combined breathing method group(Group B)and the IC-ECG combined head-turning jaw-pressing shoulder method(Group C).The catheter tip in place rate,ectopic rate,patient comfort and operator's sense of certainty of catheter placement were compared.Results The difference between the rate of in place and ectopic rate of the three groups was statistically signifi cant(P<0.05),with the highest rate of in place and the lowest rate of ectopic rate in group B,while the lowest rate of in place and the highest rate of ectopic rate in group A.The difference between the three groups was statistically signifi cant(P<0.05).The difference in the comparison of operator's affi rmation rate among the three groups was statistically signifi cant(P<0.05),in which Group C had the highest affi rmation rate and Group A had the lowest.The difference was statistically signifi cant(P<0.05)when comparing the comfort level of the patients in the three groups;with Group B having the highest level of comfort and Group C the lowest.Conclusion The application of IC-ECG combined with respiratory cooperation method in the process of PICC catheter placement in oncology patients can improve the rate of PICC catheter tip in place,reduce the incidence of catheter ectasia in the process of catheter placement,and improve the comfort of the patients with catheter placement as well as the sense of certainty of the catheter placement of the catheter placement operator.
