基于临床调查的肺结节常见证候主次症判定 被引量:2

Diagnosis on prime and minor symptoms of common syndromes of pulmonary nodules based on clinical investigation

摘要 目的:探讨肺结节中医常见证候主次症,为肺结节证候诊断标准的建立提供依据。方法:2019年3月—2021年3月临床调查河南中医药大学第三附属医院746例肺结节患者的临床资料并建立数据库,运用描述统计、Logistic回归和数据挖掘(人工神经网络)分析常见证候的症状并判定主次症。结果:肺气虚证主症为神疲、乏力、气短、易感冒等,次症为咳嗽、纳呆、脉沉;肺脾气虚证主症为神疲、乏力、恶风、面色萎黄、大便溏等,次症为咳嗽、痰色白、肢体困倦等;痰浊阻肺证主症为咳嗽、痰色白、苔白腻、脉滑,次症为纳呆、气短、痰黏、咽痒等;血瘀证主症为舌质暗、舌有瘀斑,次症为气短、纳呆、脉弦等。结论:初步判定肺结节肺气虚证、肺脾气虚证、痰浊阻肺证和血瘀证常见证候的主次症,为证候诊断标准的建立提供依据。 Objective:To explore the prime and minor symptoms of common syndromes of pulmonary nodules,and provide a basis for the establishment of diagnostic criteria for syndromes of pulmonary nodules.Methods:From March 2019 to March 2021,clinical data of 746 patients with pulmonary nodules from Third Affiliated Hospital of Henan University of Traditional Chinese Medicine were investigated and a database was established.Descriptive statistics,Logistic regression,and data mining(artificial neural network)were used to analyze the symptoms of common syndromes and determine the prime and minor symptoms.Results:The prime symptoms of lung qi deficiency syndrome are mental fatigue,lack of strength,shortness of breath,susceptibility to colds,etc.,and the minor symptoms are cough,dizziness,and pulse sinking;The prime symptoms of lungspleen qi deficiency syndrome include mental fatigue,lack of strength,aversion to wind,sallow complexion,and loose stools,followed by cough,white phlegm,and limb fatigue,etc.;The prime symptoms of phlegm turbidity obstructing lung syndrome are cough,white phlegm,white and greasy fur,and smooth pulse,and the minor symptoms are dizziness,shortness of breath,sticky phlegm,pharyngeal itching,etc.;The prime symptoms of blood stasis syndrome are dark tongue texture and ecchymosis on the tongue,followed by shortness of breath,dizziness,pulse string,etc..Conclusion:Preliminary determination of the prime and minor symptoms of common syndromes such as lung qi deficiency,lung-spleen qi deficiency,phlegm turbidity obstructing lung,and blood stasis in pulmonary nodules provides a basis for the establishment of diagnostic criteria for syndromes.
