摘要
目的:探讨掌侧入路锁定钢板内固定术治疗桡骨远端骨折的效果及对腕关节功能的影响。方法:选取2020年1月—2021年1月中国人民解放军陆军第八十集团军医院收治的106例桡骨远端骨折患者为研究对象,使用随机数表法将其分为观察组及对照组,各53例。两组均接受锁定钢板内固定术治疗,对照组在术中采用背侧入路,观察组在术中采用掌侧入路。比较两组的手术相关指标、腕关节活动范围、微循环状态指标及并发症发生率。结果:两组手术时间、术中出血量、骨折愈合时间比较,差异无统计学意义(P>0.05)。术后1周,两组腕关节背伸、掌屈、桡偏、尺偏、前臂旋前和前臂旋后6项活动范围比较,差异无统计学意义(P>0.05)。术后3个月,两组腕关节各项活动范围均大于术后1周,且观察组大于对照组,差异有统计学意义(P<0.05)。术前,两组全血黏度高切、全血黏度低切、血浆黏度比较,差异无统计学意义(P>0.05);术后,两组上述各项指标均低于术前,观察组低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为1.89%,低于对照组的13.21%,差异有统计学意义(P<0.05)。结论:在锁定钢板内固定术中采用掌侧入路,有助于恢复桡骨远端骨折的腕关节功能、改善微循环状态,减少并发症的发生,安全有效。
Objective:To investigate the effect of palmar approach locking plate internal fixation in the treatment of distal radius fractures and the influence on wrist joint function.Method:A total of 106 patients with distal radius fracture who were admitted to the People's Liberation Army Ground Force Eighty Army Hospital from January 2020 to January 2021 were selected as the research objects,and they were divided into the observation group and the control group by using the random number table method,with 53 cases in each group.Both groups received locking plate internal fixation,the control group used the dorsal approach during the surgery,and the observation group used the palmar approach during the surgery.The surgical related indicators,wrist joint range of motion,microcirculation status indicators,and incidence of complications between the two groups were compared.Result:There were no statistically significant differences in surgical time,intraoperative bleeding,and fracture healing time between the two groups(P>0.05).One week after surgery,there were no statistically significant differences in the six range of motion of wrist joint dorsiflexion,palmar flexion,radial deviation,ulnar deviation,forearm pronation,and forearm pronation between the two groups(P>0.05).Three months after surgery,the range of wrist joint activity in both groups were greater than those one week after surgery,and the indexes in the observation group were greater than those in the control group,the differences were statistically significant(P<0.05).Before surgery,there were no statistically significant differences in the levels of whole blood viscosity high shear,whole blood viscosity low shear,plasma viscosity between the two groups(P>0.05).After surgery,the above indicators in both groups were lower than those before surgery,and the indexes in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 1.89%,which was lower than 13.21%of the control group,the difference was statistically significant(P<0.05).Conclusion:The use of palmar approach locking plate internal fixation can help restore wrist joint function,improve microcirculation,reduce complications,and be safe and effective in distal radius fractures.
