药学干预下不同肠内营养配方对重症患者营养状况和不良反应的影响

Effect of Nutritional Condition and Adverse Reactions of Different Enteral Nutrition Formulas in Critically Ill Patients under Pharmaceutical Intervention

摘要 目的:探讨药学干预下不同肠内营养配方对重症患者营养状况和不良反应的影响。方法:选取2021年8月—2022年12月广东省工伤康复医院收治住院且需要营养支持(enteral nutrition,EN)治疗的60例重度颅脑损伤或重度脊髓损伤患者。采用随机数表法将其分为研究组和对照组,各30例。在药学干预下,研究组给予肠内营养混悬液联合整蛋白型肠内营养剂治疗,对照组给予单独肠内营养混悬液治疗,均治疗2周。比较两组营养指标、免疫指标、炎症水平及不良反应发生率。结果:治疗后,两组血清总白蛋白(total protein,TP)、血清白蛋白(albumin,ALB)水平均高于治疗前,且研究组TP和ALB水平均高于对照组,差异有统计学意义(P<0.05)。治疗后,两组免疫球蛋白G(immunoglobulin G,IgG)水平均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05)。治疗后,两组降钙素原(procalcitonin,PCT)、C反应蛋白(C-reactive protein,CRP)和超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)均低于治疗前,且研究组均低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:药学干预下,肠内营养混悬液联合整蛋白型肠内营养剂能够有效均衡营养供给,不增加不良反应,利于改善重症患者营养状况。 Objective:To explore the effect of nutritional condition and adverse reactions of different enteral nutrition formulas in critically ill patients under pharmaceutical intervention.Method:A total of 60 patients with severe craniocerebral or severe spinal cord injury who were hospitalized in Guangdong Work Injury Rehabilitation Hospital and in need of enteral nutrition(EN)treatment from August 2021 to December 2022 were selected.The patients were divided into study group and control group,with 30 cases in each group.Under pharmaceutical intervention,the study group was treated with Enteral Nutritional Suspension combined with Intacted Protein Enteral Nutrition Powder,and the control group was treated with Enteral Nutritional Suspension alone for 2 weeks.The nutritional indexes,immune indexes,inflammatory levels and the incidence of adverse reactions were compared between the two groups.Result:After treatment,serum total protein(TP)and serum albumin(ALB)levels in both groups were higher than those before treatment,and the levels of TP and ALB in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).After treatment,immunoglobulin G(IgG)level in both groups were higher than those before treatment,and that in the study group was higher than that in the control group,the differences were statistically significant(P<0.05).After treatment,procalcitonin(PCT),C-reactive protein(CRP)and hypersensitive C-reactive protein(hs-CRP)in both groups were lower than those before treatment,those in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).The difference was not statistically significant in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Under pharmaceutical intervention,Enteral Nutritional Suspension combined with Intacted Protein Enteral Nutrition Powder can effectively balance nutritional supply without increasing incidence of adverse reactions,which is beneficial to improve the nutritional status of critically ill patients.
