中药治疗慢性心力衰竭用药规律及可能机制的研究 被引量:2

Research on the Medication Rules and Possible Mechanisms of Traditional Chinese Medicine Treatment of Chronic Heart Failure

摘要 目的基于数据挖掘方法总结中药治疗慢性心力衰竭(chronic heart failure,CHF)的用药规律,利用网络药理学方法对核心药物组合治疗CHF的活性成分和作用机制进行初步预测。方法收集2014年1月至2021年6月中国中医科学院西苑医院第一诊断为慢性心力衰竭的住院患者的病历,按照纳入和排除标准,利用中医传承辅助平台(Traditional Chinese Medicine Inheritance Computer System,TCMICS)V3.0对中药使用频次、用药规律等进行分析;采用网络药理学的方法,筛选出核心药物组合治疗CHF的潜在靶点,初步预测其可能的作用机制。结果共纳入625例患者,787张中药处方。有56味中药使用频次超过50次,主要为补虚药、利水渗湿药及活血化瘀药;结合高频中药、关联规则及聚类分析得到治疗CHF的核心药物组合为:黄芪、党参、茯苓、桂枝、益母草、川芎和赤芍。网络药理学分析表明核心药物组合中槲皮素、异鼠李素等活性成分作用于AKT1、EGFR、MMP9等20个CHF核心靶点;核心药物组合治疗CHF的潜在靶点主要富集在11个细胞组成、17种分子功能和20个生物过程中;生物学通路主要为MAPK信号通路、PI3K-Akt信号通路、AMPK信号通路等。结论中药治疗CHF以益气温阳,利水消肿,活血化瘀为主。核心药物组合可能通过参与心室重塑、炎症反应等过程从而发挥治疗CHF的作用。 Objective To summarize the medication rules of traditional Chinese medicine(TCM)in the treatment of chronic heart failure(CHF)based on data mining technology,and to preliminarily predict the active components and mechanisms of core drug combination in the treatment of CHF by using network pharmacology technology.Methods We collected the medical records of inpatients with CHF who were first diagnosed in Xiyuan Hospital of China Academy of Chinese Medical Sciences from January 2014 to June 2021 according to the inclusion and exclusion criteria,and then analyzed the frequency and regularity of the use of TCM by using the Traditional Chinese Medicine Inheritance Computer System(TCMICS)V3.0.Using network pharmacology method,we screened out potential targets of core drug combinations for CHF and preliminarily predicted their possible mechanisms.Results A total of 625 patients and 787 prescriptions of TCM were included.There were 56 herbs were used more than 50 times,which mainly used for supplementing deficiency,promoting diuresis and removing blood stasis.The core drug combination for the treatment of CHF,including Astragalus Radix,Codonopsis Radix,Poria,Cinnamomi Ramulus,Leonuri Herba,Chuanxiong Rhizoma and Paeoniae Radix Rubra,was obtained by the combination of frequency analysis,association rules and cluster analysis.Network pharmacology showed that quercetin,isorhamnetin and other active components in the core drug combination acted on 20 CHF core targets such as AKT1,EGFR,MMP9,etc.The potential targets of core drug combination for the treatment of CHF are mainly concentrated in 11 cell components,17 molecular functions and 20 biological processes.Biological pathways mainly include MAPK signaling pathway,PI3K Akt signaling pathway,AMPK signaling pathway,etc.Conclusion TCM is mainly used to treat CHF by supplementing qi and warming yang,promoting water and detumescence,promoting blood circulation and removing blood stasis.The core drug combination may play a role in the treatment of CHF by participating in the process of ventricular remodeling,inflammatory reaction,etc.
