摘要
目的分析达格列净联合缬沙坦治疗老年早期2型糖尿病肾病的效果及对血糖水平的影响。方法抽取2019年4月至2022年4月联勤保障部队第九八八医院焦作院区收治的老年早期2型糖尿病肾病患者60例,按随机数字表法分为对照组与观察组,每组30例。对照组采用二甲双胍联合缬沙坦治疗,观察组采用达格列净联合缬沙坦治疗。比较两组临床疗效及不良反应发生情况。比较两组治疗前后肾功能指标[尿素氮(BUN)、尿微量白蛋白/肌酐(ACR)]水平、血糖水平[糖化血红蛋白(HbA1c)、空腹血糖(FPG)、餐后2 h血糖(2hPBG)]及内皮素-1水平。结果观察组总有效率(96.67%,29/30)高于对照组(73.33%,22/30),P<0.05。治疗后,观察组BUN、ACR水平低于对照组(P<0.05)。治疗后,观察组HbA1c、FPG、2hPBG水平低于对照组(P<0.05)。治疗后,观察组内皮素-1水平低于对照组(P<0.05)。观察组不良反应发生率(3.33%,1/30)与对照组(16.67%,5/30)比较差异未见统计学意义(P>0.05)。结论达格列净联合缬沙坦治疗老年早期2型糖尿病肾病患者,能降低血糖水平,改善肾功能和血管内皮功能,临床效果和安全性理想。
Objective To analyze the effect of dapagliflozin combined with valsartan in the treatment of early type 2 diabetic nephropathy in the elderly,and its influence on blood glucose level.Methods Sixty elderly patients with early type 2 diabetic nephropathy treated in Jiaozuo Branch of No.988 Hospital of the Joint Logistic Support Force from April 2019 to April 2022 were selected,and they were divided into control group and observation group according to random number table method,with 30 cases in each group.The control group was given metformin combined with valsartan,and the observation group was given dapagliflozin combined with valsartan.The treatment efficacy and incidence of adverse reactions of the two groups were compared.The renal function assessed by blood urea nitragen(BUN)and albumin/urine creatinine ratio(ACR),levels of blood glucose assessed by glycosylated hemoglobin(HbA1c),fasting plasma glucose(FPG)and 2-hour postprandial blood glucose(2hPBG),level of endothelin-1 of the two groups were compared before and after treatment.Results The total effective rate of the observation group(96.67%,29/30)was higher than that of the control group(73.33%,22/30),P<0.05.After treatment,the levels of BUN and ACR in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of HbA1c,FPG and 2hPBG in the observation group were lower than those in the control group(P<0.05).After treatment,endothelin-1 level in the observation group was lower than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the observation group(3.33%,1/30)and the control group(16.67%,5/30),P>0.05.Conclusions Dapagliflozin combined with valsartan in the treatment of elderly patients with early type 2 diabetic nephropathy can reduce blood glucose level,improve renal function and vascular endothelial function,with ideal clinical effect and safety.
