超声乳化人工晶状体植入术治疗高度近视白内障患者临床疗效观察

The effects of phacoemulsification and intraocular lens implantation on visual function,anterior segment parameters and corneal endothelial function in patients with high myopia cataract

摘要 目的探讨超声乳化人工晶状体(IOL)植入术对高度近视白内障(HMC)患者视功能、眼前节参数和角膜内皮功能的影响。方法将我院收治的103例(103眼)HMC患者依照数字奇偶法随机分为两组,研究组(n=52,52眼)采用超声乳化IOL植入术,白内障囊外摘除术组(ECCE组)(n=51,51眼)采用ECCE,比较两组患者术后3个月视功能、眼前节参数和角膜内皮功能的变化及不良反应发生情况。结果研究组治疗总有效率94.23%高于ECCE组80.39%(P<0.05),术后视力矫正改善优于ECCE组(P<0.05);术后3个月,两组患者复查患眼时前房深度与角度均升高(P<0.05),研究组变化幅度高于ECCE组(P<0.05);中央角膜厚度研究组与术前比较差异无统计学意义(P>0.05),ECCE组则低于术前(P<0.05),两组变化幅度差异有统计学意义(P<0.05)。两组屈光度均降低,研究屈光度改善幅度大于ECCE组(P<0.05);两组角膜内皮细胞密度、眼轴长度均降低,研究组降低幅度低于ECCE组(P<0.05);两组角膜曲度虽均较术前降低,但组间比较差异无统计学意义(P>0.05);研究组术后并发症发生率3.85%低于ECCE组15.69%(P<0.05)。结论对HMC患者实施超声乳化IOL植入术治疗临床效果良好,可有效提高其视功能与角膜内皮功能,改善其眼前节参数,且较为安全。 Objective To investigate the influence of phacoemulsification with intraocular lens(IOL)implantation on the visual function,anterior segment parameters and corneal endothelial function in the treatment of patients with high myopia cataract(HMC).Methods One hundred and three patients(103 affected eyes of severe side)with HMC were enrolled.The patients were randomly classified into a study group(n=52,52 eyes)and an extracapsular cataract extraction group(ECCE group,n=51,51 eyes)by means of the odd-even number method.The study group was treated with phacoemulsification with IOL implantation and the ECCE group was treated with extracapsular cataract extraction.The visual function,anterior segment parameters and corneal endothelial function as well as incidence of adverse reactions were compared after 3 months of operation between the two groups.Results The total effective rate of treatment in the study group was 94.23%,that was higher than 80.39%in the ECCE group(P<0.05).The number of cases with improved postoperative visual acuity correction in the study group was more than that in the ECCE group(P<0.05).After 3 months of surgery,the anterior chamber depth and angle of the affected eye were increased in both groups(P>0.05),but the changes in the study group were higher than those in the ECCE group(P<0.05).The central corneal thickness in the study group was not different from that before surgery(P<0.05),but the thickness in the ECCE group was thinner than that before surgery(P<0.05),and there was a significant difference in the change between the two groups(P<0.05).The diopter in both groups was decreased after 3 months of surgery,but the improvement of diopter was greater in the study group than that in the ECCE group(P<0.05).The corneal endothelial cell density and axial length in the two groups were reduced after 3 months of surgery,but the reductions in the study group were lower than that in the ECCE group(P<0.05).The corneal curvature in the two groups after 3 months of surgery was lower than that before surgery,but there was no difference between the two groups(P>0.05).After 3 months of surgery,the total incidence rate of complications in the study group was 3.85%,that was lower than 15.69%in the ECCE group(P<0.05).Conclusions Phacoemulsification with IOL implantation has a good clinical effect in the treatment of patients with HMC.It can effectively enhance the visual function and corneal endothelial function as well as improve the anterior segment parameters.It is relatively safe.
