摘要
目的探析胫骨平台骨折(高能量)开展微创钢板固定的治疗效果。方法选取2019年4月—2023年3月上海市静安区闸北中心医院收治的60例高能量胫骨平台骨折患者为研究对象,采用随机数表法分为两组,每组30例。对照组采用传统切开复位内固定术,观察组采用微创钢板固定法,比较两组手术并发症、膝关节功能、手术情况。结果观察组术后并发症率为3.33%低于对照组的20.00%,差异有统计学意义(χ^(2)=4.043,P<0.05)。观察组切口长度、出血剂量、术后影像学检查提示骨折线模糊时间均优于对照组,差异有统计学意义(P<0.05)。术前,两组膝关节功能评分、活动度比较,差异无统计学意义(P>0.05);术后,观察组关节(内、外、屈曲)各活动角度大于对照组,膝关节评估分数高于对照组,差异有统计学意义(P<0.05)。结论高能量胫骨平台骨折患者采用微创钢板固定法治疗的切口更小,创伤性更低,术后可加速膝关节功能恢复,提高关节活动度。
Objective To explore the therapeutic effect of carrying out minimally invasive plate fixation for tibial plateau fracture(high-energy).Methods Sixty patients with high energy tibial plateau fracture admitted to Zhabei Central Hospital,Jing'an District,Shanghai from April 2019 to March 2023 were selected as the study objects,and were divided into two groups by random number table method,with 30 patients in each group.The control group was treated with traditional open reduction and internal fixation,and the observation group was treated with minimally invasive plate fixation.The surgical complications,knee joint function and operation status were compared between the two groups.Results The postoperative complication rate of the observation group(3.33%)was lower than that of the control group(20.00%),and the difference was statistically significant(χ^(2)=4.043,P<0.05).Incision length,bleeding dose and postoperative imaging examination in the observation group were better than those in the control group,the difference was statistically significant(P<0.05).Before operation,there was no statistically significant difference in knee function score and range of motion between the two groups(P>0.05).After operation,the movement angles of the joints(internal,external and flexion)in the observation group were higher than those in the control group,and the evaluation scores of the knee joint were higher than those in the control group,the difference was statistically significant(P<0.05).Conclusion Patients with high-energy tibial plateau fracture treated with minimally invasive plate fixation have smaller incisions and less trauma,which can accelerate the functional recovery of knee joint and improve joint motion after surgery.
