摘要
目的探讨特应性哮喘和非特应性哮喘患儿血清总免疫球蛋白E(IgE)水平、呼出气一氧化氮(FeNO)水平和外周血嗜酸性粒细胞(EOS)的相关性。方法收集我院儿科特应性哮喘患儿105例、非特应性哮喘患儿84例以及对照患儿80例。通过采用化学发光法检测血清总IgE水平、使用NO分析仪检测FeNO水平及通过自动血细胞分析仪检测外周血EOS计数和比例。比较各组血清总IgE水平、FeNO水平及外周血EOS计数和比例的差异。Logistic回归分析特应性哮喘的影响因素,受试者工作特征(ROC)曲线分析3个指标对特应性哮喘的预测价值。结果与对照组和非特应性哮喘组相比,特应性哮喘组血清总IgE水平、FeNO水平及外周血EOS计数和比例升高(P<0.05)。Logistic回归分析结果显示,外周血EOS比例(OR=1.259,95%CI:1.039~1.525)、血清总IgE水平(OR=1.003,95%CI:1.001~1.005)、FeNO水平(OR=1.087,95%CI:1.019~1.158)升高是特应性哮喘患儿发病的影响因素。ROC曲线结果显示,外周血EOS比例(AUC=0.789,95%CI:0.710~0.869)、血清总IgE(AUC=0.743,95%CI:0.657~0.828)、FeNO(AUC=0.702,95%CI:0.615~0.790)对于特应性哮喘患儿均具有一定的鉴别诊断价值,但三者联合的诊断效能更高(AUC=0.854,95%CI:0.792~0.917)。结论联合检测血清总IgE水平、FeNO水平及外周血EOS有助于提高特应性哮喘患儿的诊断准确率。
Objective To investigate the correlation between serum total immunoglobulin E(IgE)level,exhaled nitric oxide(FeNO)level,and peripheral blood eosinophil(EOS)in children with atopic asthma and non-atopic asthma.Methods 105 children with atopic asthma,84 children with non-atopic asthma,and 80 control cases were collected in our hospital.Serum total IgE level was detected by chemiluminescence method,FeNO level was detected by NO analyzer,and peripheral blood EOS count and proportion was detected by automatic blood cell counting analyzer.The serum total IgE level,FeNO level,and peripheral blood EOS count were compared among all groups.The influencing factors of atopic asthma were analyzed by Logistic regression,and the predictive value of three indexes in atopic asthma was analyzed by the receiver operating characteristic(ROC)curve.Results Compared with the control group and non-atopic asthma group,the serum total IgE level,FeNO level,and peripheral blood EOS count and proportion in the atopic asthma group were increased(P<0.05).Logistic regression analysis showed that peripheral blood EOS proportion(OR=1.259,95%CI:1.039-1.525),serum total IgE level(OR=1.003,95%CI:1.001-1.005),and FeNO level(OR=1.087,95%CI:1.019-1.158)were influential factors in the pathogenesis of children with atopic asthma.ROC curve results showed that peripheral blood EOS proportion(AUC=0.789,95%CI:0.710-0.869),serum total IgE(AUC=0.743,95%CI:0.657-0.828),and FeNO(AUC=0.702,95%CI:0.615-0.790)had certain value in the differential diagnosis of children with atopic asthma,but the combined efficacy of the three was higher(AUC=0.854,95%CI:0.792-0.917).Conclusion The combined detection of serum total IgE level,FeNO level,and peripheral blood EOS can improve the diagnostic accuracy of children with atopic asthma.
