92例烧伤合并脓毒症患者主要病原菌构成及耐药性探究 被引量:1

摘要 目的研究92例烧伤合并脓毒症患者主要病原菌构成及耐药性。方法此次纳入的研究对象:院内收治的烧伤合并脓毒症患者(样本量:92例),选例地点:郑州市第一人民医院烧伤科,选取时间:2018年12月至2021年12月,统计患者主要病原菌分布及主要菌株的耐药性(革兰阴性菌、革兰阳性菌)。结果92例烧伤合并脓毒症患者中,共检出126株病原菌,其中鲍曼不动杆菌、铜绿假单胞菌、肺炎克雷伯菌是102株(80.95%)革兰阴性菌中的主要菌株;金黄色葡萄球菌是15株(11.90%)革兰阳性菌中的主要菌株;白假丝酵母菌是9株(7.14%)真菌中的主要菌株;鲍曼不动杆菌对庆大霉素、复方新诺明等的耐药率较高;铜绿假单胞菌对庆大霉素、妥布霉素等的耐药率较高;肺炎克雷伯菌对庆大霉素、复方新诺明等的耐药率较高;金黄色葡萄球菌对头孢西丁、青霉素等的耐药率较高。结论临床上在对烧伤合并脓毒症患者病原菌的分析过程中能够发现主要的菌种为革兰阴性菌,并且其对于不同药物有着不同的耐药性,基于此可指导患者选取耐药性较低的药物治疗,以达到改善预后的目的。 Objective To explore the main pathogenic bacteria composition and drug resistance in 92 burn patients with sepsis.Methods ASubjects included in this study:burn patients complicated with sepsis admitted in the hospital(sample size:92 cases).The selected case was located in the Department of Burns of Zhengzhou First People's Hospital,and the selected time was from December 2018 to December 2021.and the distribution of main pathogenic bacteria and drug resistance of main strains(gram-negative bacteria and gram-positive bacteria)were counted.Results Among the 92 burn patients with sepsis,a total of 126 pathogenic bacteria were detected,Acinetobacter baumannii,Pseudomonas aeruginosa and Klebsiella pneumoniae were the main strains among 102(80.95%)gram-negative bacteria.Staphylococcus aureus was the main strain among 15(11.90%)gram-positive bacteria.Candida albicans was the main strain of 9 fungi(7.14%).The resistance rate of Acinetobacter baumannii to gentamicin and cotrimoxazole was high;The resistance rate of Pseudomonas aeruginosa to gentamicin and tobramycin was high;The resistance rate of Klebsiella pneumoniae to gentamicin and cotrimoxazole was high;The resistance rate of Staphylococcus aureus to cefoxitin and penicillin was high.Conclusion In the clinical analysis of pathogenic bacteria of burn patients complicated with sepsis,it can be found that the main bacteria are gram-negative bacteria.And it has different drug resistance to different drugs.Based on this,it can guide patients to choose drugs with lower drug resistance to improve the prognosis.
