摘要
目的:分析腹腔镜下经腹腹膜前疝修补术(TAPP)和腹腔镜全腹膜外补片修补术(TEP)对男性腹股沟疝睾酮浓度和金属蛋白酶组织抑制物-1(TIMP-1)的影响。方法:选取厦门市第五医院2022年1月—2023年3月收治的80例男性腹股沟疝患者,按照随机数字表法分为研究组与对照组,研究组(40例)采用TEP治疗,对照组(40例)采用TAPP治疗,对比两组临床效果。结果:研究组精索动脉管径小于对照组,睾丸动脉舒张末期血流速度(EDV)、睾酮浓度和TIMP-1数值均高于对照组,同时术后视觉模拟评分法(VAS)评分、并发症总发生率及复发率均低于对照组,术中出血量少于对照组,手术时间和首次下床活动时间均短于对照组(P<0.05)。结论:TEP在男性腹股沟疝的临床治疗中效果更好,能够减少手术对患者生殖功能的影响,降低术后复发疝发生风险,预后良好,治疗实用性较高。
Objective:To analyze the effects of endoscopic transabdominal preperitoneal prosthesis(TAPP)and endoscopic total extraperitoneal prosthesis(TEP)on testosterone concentration and tissue inhibitor of metalloproteinase-1(TIMP-1)in male inguinal hernia.Method:A total of 80 male patients with inguinal hernia admitted to Xiamen Fifth Hospital from January 2022 to March 2023 were selected and divided into the study group and the control group according to the random number table method.The study group(40 cases)was treated with TEP,and the control group(40 cases)was treated with TAPP.The clinical efficacy of the two groups were compared.Result:The spermatic artery diameter of the study group was smaller than that of the control group,and the testicular artery end-diastolic blood flow velocity(EDV),testosterone concentration and TIMP-1 value were higher than those of the control group,the postoperative visual analogue scale(VAS)score,the total incidence of complications and recurrence rate were lower than those of the control group,and the intraoperative blood loss was less than that of the control group,the operation time and first ambulation time of the study group were shorter than those of the control group(P<0.05).Conclusion:TEP is more effective in the clinical treatment of male inguinal hernia,which can reduce the impact of surgery on the reproductive function of patients,reduce the risk of recurrent hernia after surgery,improve the prognosis and have high practicality.
