摘要
目的:分析不同免疫状态侵袭性肺曲霉菌病患者实验室指标表达状况。方法:选取树兰(杭州)医院呼吸与危重症医学科病房2020年1月至2022年12月收治的44例侵袭性肺曲霉菌病患者,评价患者的基础免疫状况,根据免疫状态分为两组:免疫低下组(n=27)、免疫正常组(n=17),对比不同免疫状态侵袭性肺曲霉菌病患者实验室指标表达情况。结果:经分析不同免疫状态侵袭性肺曲霉菌病患者实验室指标发现:与免疫正常组相比,免疫低下组CD3^(+)T细胞、CD4^(+)/CD8^(+)T细胞减少,IFN-γ、IL-6、IL-10、C反应蛋白(CRP)、降钙素原(PCT)、CD8^(+)T细胞、半乳甘露聚糖(GM)、(1,3)-β-D-葡聚糖(BG)水平升高(P<0.05)。结论:不同免疫状态侵袭性肺曲霉菌病患者临床表现不同,免疫低下患者临床症状/体征主要表现为咳嗽、发热、湿啰音等,实验室指标较免疫正常患者显著异常,患者预后大多不良,治愈率较低,且患者预后不良受多种因素共同影响,临床应针对不同免疫状态患者的疾病特征和预后状况,采用有效的措施干预,以促进患者预后恢复。
Objective:To analyze expressions of laboratory indexes in invasive pulmonary Aspergillus patients with different immune status.Methods:A total of 44 patients with invasive pulmonary Aspergillus disease from the respiratory and critical care wards of Shulan(Hang Zhou)Hospital from January 2020 to December 2022 were selected to evaluate basic immune status of the patients.According to immune status,patients were divided into two groups:immunocompromised group(n=27)and normal immune group(n=17).Compared expressions of laboratory indexes in patients with invasive pulmonary Aspergillus with different immune status.Results:Laboratory indexes of patients with invasive pulmonary Aspergillus in different immune states were analyzed:compared with normal immunologic group,CD3+T cells,CD4^(+)/CD8^(+)T cells were decreased,while IFN-γ,IL-6,IL-10,C-reactive protein(CRP),procalcitonin(PCT),CD8^(+)T cells,galactomannan(GM)and(1,3)-beta-d-glucan(BG)levels were increased in immunocompromised group(P<0.05).Conclusion:Clinical manifestations of patients with invasive pulmonary Aspergillus are different in different immune states.Clinical symptoms/signs of patients with immunosuppression mainly include cough,fever,moist rales,etc.,laboratory indicators of patients with immunosuppression are significantly abnormal compared with those of patients with normal immunity.Prognosis of patients is mostly poor with low cure rate,and the poor prognosis of patients are affected by various factors.In order to promote the recovery of patients'prognosis,reasonable and effective interventions should be actively adopted according to the disease characteristics and prognosis of patients with different immune states.
