摘要
目的剖析三种化疗方案VD､PAD､VRD治疗多发性骨髓瘤(multiple myeloma,MM)的效果,为临床选择合理的化疗方案提供建议。方法选择2019年3月—2022年3月甘肃宝石花医院收治的MM患者95例,根据选用化疗方法的不同,分为VD组27例､PAD组35例和VRD组33例,VD组采用硼替佐米+地塞米松的化疗方案,PAD组采用硼替佐米+阿霉素+地塞米松的化疗方案,VRD组采用硼替佐米+来那度胺+地塞米松的化疗方案;对比三组MM化疗方案应用效果,分析三种方案的优缺点。结果VD､PAD和VRD方案,均可有效缓解和治疗MM;3药联合方案PAD､VRD优于2药方案VD;PAD和VRD方案,疗效相当。结论在兼顾患者体况､疗效和经济因素的原则下,对于基础评估心脏功能不全､器官功能低下的患者,可选用VD方案化疗;一般器官功能良好､可行诱导后移植的患者,可选用PAD方案化疗;心脏功能不全､不能耐受联合化疗､拟行诱导后移植的患者,可选用VRD方案化疗,该方案化疗不良反应更小。
Objective To analyze the application effect of three chemotherapy regimens VD,PAD and VRD in the treatment of multiple myeloma(MM),and to provide suggestions for clinical reasonable chemotherapy regimens.Methods Ninety-five MM patients admitted to Gansu Baoshihua Hospital from March 2019 to March 2022 were selected.According to the different chemotherapy methods,all patients were divided into VD group(27 cases),PAD group(35 cases)and VRD group(33 cases).The VD group,PAD group,and VRD group adopted Bortezomib&Dexamethasone chemotherapy regimens,Bortezomib&Adriamycin&Dexamethasone chemotherapy regimens,and Bortezomib&Thalidomide&Dexamethasone chemotherapy regimens,respectively.The clinical effects of the three groups were compared,and the advantages and disadvantages of the three chemotherapy regimens were analyzed.Results VD,PAD and VRD regimens can effectively alleviate and treat MM.PAD and VRD regimens were better than VD regimen.PAD and VRD regimens have similar efficacy.Conclusion Under the principle of considering the patient’s physical condition,curative effect and economic factors,VD regimen can be used for patients with cardiac insufficiency and poor organ function.PAD regimen can be used for patients with cardiac insufficiency,good economy,unable to tolerate combined chemotherapy,and planning to undergo transplantation after induction.VRD chemotherapy can be used for patients with good organ function and feasible transplantation after induction.
