膝关节镜术后镇痛的研究进展

摘要 随着手术技术的进步和新型关节镜设备的出现,关节镜微创技术在膝关节手术中得到了广泛应用。膝关节镜手术相较于开放手术大大减少了手术创伤和术后疼痛,但由于器械对关节组织的损伤和冲洗液对关节腔的刺激等因素,术后仍然会出现不同程度的疼痛。目前有多种方法可用于膝关节镜术后疼痛控制,包括关节周围局部浸润镇痛、外周神经阻滞、全身应用药物镇痛、椎管内镇痛、多模式镇痛等。选择合适的术后镇痛方式,促进术后早期功能锻炼已成为麻醉医生的关注热点,因此本文就膝关节镜术后各种镇痛方式及其优缺点进行综述。 With the progress of surgical technology and the emergence of new arthroscopic equipment,minitrauma arthroscopic technique has been widely used in knee surgery.Compared with open surgery,arthroscopic knee surgery greatly reduces surgical trauma and postoperative pain.However,due to the injury of joint tissue caused by instruments and stimulation of joint cavity caused by washing fluid,different degrees of postoperative pain may still occur.At present,there are many methods for pain control after arthroscopic knee surgery,including periarticular local infiltration analgesia,peripheral nerve block,intravenous analgesia,intrathecal analgesia and multimodal analgesia.Selecting appropriate postoperative analgesia and promoting early postoperative functional exercise have become the focus of anesthesiologists.Therefore,this paper reviews the various analgesia methods after knee arthroscopy and their respective advantages and disadvantages.
