摘要 目的:探讨尿路感染诊断中应用尿常规检验的价值。方法:选取2021年2月—2022年1月东莞市黄江医院收治的尿路感染患者(研究组)43例及同期健康体检者(对照组)43例作为研究对象。两组均进行尿常规检验和细菌培养,比较两组细菌计数、白细胞数量、亚硝酸盐阳性率,分析尿路感染检出的病原菌及尿常规检验准确性。结果:研究组细菌计数和白细胞数量均高于对照组,差异有统计学意义(P<0.001);研究组共检出46株病原菌,其中革兰阳性菌13株(占比28.26%),革兰阴性菌30株(占比65.22%),真菌3株(占比6.52%);革兰阳性菌中粪肠球菌占比最高,革兰阴性菌中大肠埃希菌占比最高。细菌计数阳性预测值91.4%、敏感度66.7%、特异度75.0%;白细胞计数阳性预测值88.6%、敏感度75.0%、特异度62.5%;亚硝酸盐阳性预测值91.7%、敏感度75.0%、特异度71.4%。结论:尿常规检验细菌计数、白细胞数量、亚硝酸盐等相关指标在尿路感染诊断中具有较高的临床价值,可为临床治疗方案提供可靠依据。 Objective:To investigate the value of routine urine test in the diagnosis of urinary tract infection.Methods:43 patients with urinary tract infections admitted to Dongguan Huangjiang Hospital from February 2021 to January 2022(the study group)and 43 patients with health checkups during the same period(the control group)were selected as the study subjects.Both groups were subjected to routine urinalysis and bacterial culture,so that the indexes such as microscopic bacterial counts,leukocyte counts,and nitrite positivity rates of the two groups were compared.Results:The microscopic bacterial count and leukocyte count of the study group were higher than those of the control group,and the differences were statistically significant(P<0.001);46 case out of all study subjects were detected with pathogenic bacteria,including 13 Gram-positive bacteria(28.26%),30 Gram-negative bacteria(65.22%),and 3 fungi(6.52%);Enterococcus faecalis had the highest percentage of Gram-positive bacteria,and Escherichia coli had the highest percentage of Gram-negative bacteria.The positive predictive value of microscopic bacterial count was 91.4%,sensitivity 66.7%,and specificity 75.0%;the positive predictive value of white blood cell count was 88.6%,sensitivity 75.0%,and specificity 62.5%;and the positive predictive value of nitrite was 91.7%,sensitivity 75.0%,and specificity 71.4%.Conclusion:Urinary routine examination of bacterial count,white blood cell count,nitrite and other related indicators have high clinical value in the diagnosis of urinary tract infection,and can provide a reliable basis for clinical treatment.
