摘要
目的:探讨老年机械通气患者胃肠道功能及营养状态与撤机失败的相关性。方法:选取92例老年机械通气患者为研究对象,所有受试对象均行自主呼吸试验(SBT),依据患者撤机情况分为撤机成功组(n=68)及撤机失败组(n=24),收集患者一般资料、胃肠道功能及营养状态指标,对老年机械通气患者撤机失败的危险因素进行分析。结果:92例患者中撤机失败24例,撤机失败率为26.09%(24/92);单因素及多因素Logistic回归分析结果显示,多器官功能衰竭综合征(MODS)、胃肠道功能障碍、营养状况为老年机械通气患者撤机失败的独立影响因素(均P<0.05)。结论:胃肠道功能及营养状态是老年机械通气患者撤机失败的危险因素,可预测撤机难度并给予针对性治疗,为撤机策略提供科学参考,继而降低撤机失败率。
Objective:To investigate the relationship of gastrointestinal function and nutritional status with weaning failure in elderly patients with mechanical ventilation.Methods:A total of 92 elderly patients with mechanical ventilation were selected as the study subjects.All subjects underwent spontaneous breathing test(SBT),and were divided into the successful weaning group(68 cases)and the failed weaning group(24 cases)according to the outcome of weaning.General data,gastrointestinal function and nutritional status of the two groups were comparatively analyzed.Analysis of risk factors for weaning failure in elderly patients undergoing mechanical ventilation was performed.Results:The rate of weaning failure in this study was 26.09%(24/92).Univariate analysis and multivariate Logistic regression analysis found that MODS,gastrointestinal dysfunction,nutritional status were independent influencing factors of weaning failure in elderly patients with mechanical ventilation(all P<0.05).Conclusion:Gastrointestinal function and nutritional status are influencing factors of weaning failure in elderly patients with mechanical ventilation.They can help predict the difficulty of weaning and assist in targeted treatment,thereby providing a scientific basis for developing weaning strategy and reducing the failure rate of aircraft withdrawal.
