摘要
目的 探讨经鼻内镜内下壁减压术治疗Graves眼病压迫性视神经病变的临床效果。方法 选取医院2017年1月至2022年6月收治的92例Graves眼病压迫性视神经病变患者,根据随机数字表法分为研究组和常规组,各46例。研究组接受经鼻内镜内下壁减压术,常规组接受眼眶平衡减压术,比较两组治疗前后眼球突出度及视力变化情况、术中出血量和手术时间、生活质量,比较两组并发症发生情况。结果 治疗后,两组眼球突出度较治疗前下降,且研究组低于常规组(P<0.05),两组最佳矫正视力均上升,且研究组高于常规组(P<0.05)。研究组术中出血量少于常规组,手术时间短于常规组(P<0.05)。治疗后研究组社会功能、认知功能、心理状态、健康状态评分和总分均高于常规组(P<0.05)。两组并发症总发生率比较差异无统计学意义(P>0.05)。结论 经鼻内镜内下壁减压术治疗Graves眼病压迫性视神经病变可以缓解患者眼球突出问题,帮助患者更好恢复视力,减少术中出血量并缩短手术时间,提高生活质量,且安全性高。
Objective To explore the effect of transnasal endoscopic inferior wall decompression in the treatment of compressive optic neuropathy of Graves ophthalmopathy.Methods A total of 92 patients with compressive optic neuropathy of Graves ophthalmopathy treated in the hospital from January 2017 to June 2022 were randomly divided into study group and conventional group,with 46 patients in each group.The study group received transnasal endoscopic inferior wall decompression,and the conventional group received orbital balance decompression.The changes of exophthalmos and visual acuity,the amount of intraoperative bleeding and operation time,the quality of life before and 6 months after treatment and complications were compared between the two groups.Results After treatment,the degree of exophthalmos decreased than before treatment in both groups,and the degree of exophthalmos in study group was lower than that in conventional group(P<0.05),the best corrected visual acuity of the two groups increased,and that of study group was higher than that of conventional group(P<0.05).The intraoperative bleeding in study group was less than that in conventional group,and the surgical time was shorter than that in conventional group(P<0.05).After treatment,the social function,cognitive function,mental state,health state and total score in study group were higher than those in conventional group(P<0.05).There was no statistical difference in the total incidence of complications between the two groups(P>0.05).Conclusion Transnasal endoscopic inferior wall decompression in the treatment of compressive optic neuropathy of Graves ophthalmopathy can improve patients’exophthalmos,help patients recover their vision,reduce the amount of bleeding and shorten the operation time,improve the quality of life,and with high security.
