快速扩弓对上颌牙弓狭窄患儿的影响

摘要 目的:探究快速扩弓对上颌牙弓狭窄患儿牙弓长度、牙弓宽度、基骨宽度及牙槽骨厚度的影响。方法:选择2019年7月—2022年12月福建省立医院南院收治的80例上颌牙弓狭窄患儿,根据随机数表法分为两组,各40例。对照组进行慢速扩弓治疗,观察组进行快速扩弓治疗。比较两组治疗前后的上颌牙弓长度,上颌尖牙间、第一前磨牙间及第一磨牙间的牙弓宽度及基骨宽度,比较两组上颌尖牙、第一前磨牙及第一磨牙牙颈部及根中二分之一处的牙槽骨厚度。结果:治疗前,两组上颌牙弓长度比较,差异无统计学意义(P>0.05);治疗后,两组上颌牙弓长度显著大于治疗前,且观察组大于对照组,差异有统计学意义(P<0.05)。治疗前,两组上颌尖牙间、第一前磨牙间及第一磨牙间牙弓宽度比较,差异无统计学意义(P>0.05);治疗后,两组上颌尖牙间、第一前磨牙间及第一磨牙间牙弓宽度均显著大于治疗前,且观察组显著大于对照组,差异有统计学意义(P<0.05)。治疗前,两组上颌尖牙间、第一前磨牙间及第一磨牙间基骨宽度比较,差异无统计学意义(P>0.05);治疗后,两组上颌尖牙间、第一前磨牙间及第一磨牙间基骨宽度均显著大于治疗前,且观察组均显著大于对照组,差异有统计学意义(P<0.05)。治疗前及治疗后,两组上颌尖牙、第一前磨牙及第一磨牙牙颈部及根中二分之一处牙槽骨厚度比较,差异无统计学意义(P>0.05);且两组治疗后的上颌尖牙、第一前磨牙及第一磨牙牙颈部及根中二分之一处牙槽骨厚度与本组治疗前比较,差异无统计学意义(P>0.05)。结论:快速扩弓对上颌牙弓狭窄患儿牙弓长度、宽度及基骨宽度的治疗效果相对更好,且对牙槽骨厚度无明显不良影响。 Objective:To explore the effects of rapid arch expansion on arch length,arch width,basal bone width and alveolar bone thickness in children with maxillary arch stenosis.Method:A total of 80 children with maxillary arch stenosis in the South Hospital of Fujian Provincial Hospital from July 2019 to December 2022 were selected and divided into two groups according to random number table method,with 40 cases in each group.The control group received slow arch expansion treatment,and the observation group received rapid arch expansion treatment.The maxillary arch length,arch width and basal bone width between maxillary fangs,first premolars and first molars were compared between the two groups before and after treatment,and the alveolar bone thickness between maxillary fangs,first premolars and first molars were compared between the two groups.Result:Before treatment,there was no significant difference in maxillary arch length between the two groups(P>0.05).After treatment,maxillary arch length in both groups were significantly greater than those before treatment,and observation group was greater than that in the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in arch width between maxillary fangs,first premolars and first molars between the two groups(P>0.05).After treatment,the arch width between maxillary fangs,first premolars and first molars in both groups were significantly greater than those before treatment,and the observation group were significantly greater than those in the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the basal bone width between maxillary fangs,first premolars and first molars between the two groups(P>0.05).After treatment,the basal bone widths of maxillary fangs,first premolar and first molar in both groups were significantly greater than those before treatment,and the observation group were significantly greater than those in the control group,the differences were statistically significant(P<0.05).Before and after treatment,there were no significant differences in alveolar bone thickness between the two groups in the neck of the tooth and the middle half of the root of maxillary fangs,first premolar and first molar(P>0.05).There were no significant differences in alveolar bone thickness in the neck of the tooth and the middle half of the root of maxillary fangs,first premolar and first molar between the two groups after treatment compared to before treatment in this group(P>0.05).Conclusion:Rapid arch expansion has better effect on arch length,width and basal bone width in children with maxillary arch stenosis,and has no obvious adverse effect on alveolar bone thickness.
