摘要
目的:观察探讨孤立型肺内小结节(SPN)良恶性鉴别诊断过程中多层螺旋CT三维重建技术的应用价值。方法:前瞻性选取淄博市中心医院2022年1月—12月接收的SPN患者120例,根据病检结果分为良性组(n=32)、恶性组(n=88),入院之后均进行多层螺旋CT三维重建技术扫描,对比两组CT平均值、强化峰值与CT征象检出率,并根据病检结果,观察多层螺旋CT三维重建技术诊断效能。结果:恶性组CT平均值、强化峰值均高于良性组(P<0.05),两组结节最大直径差异无统计学意义(P>0.05)。恶性组胸膜凹陷征、磨玻璃征、分叶征、空泡征、血管集束征、棘突征、晕征、短毛刺征等CT征象检出率高于良性组,钙化灶、卫星灶检出率低于良性组(P<0.05)。根据病检结果,多层螺旋CT三维重建技术鉴别诊断SPN良恶性准确率为95.83%、灵敏度96.59%、特异度93.75%。结论:SPN良恶性鉴别诊断过程中多层螺旋CT三维重建技术的应用具有较高的定性诊断价值,能够全面明确病变特征及病灶形态,为临床有效诊治提供参考。
Objective To observe and explore the application value of multi-slice spiral CT three-dimensional reconstruction technology in the differential diagnosis of benign and malignant solitary pulmonary nodules(SPN).Methods A prospective selection of 120 patients with SPN admitted to Zibo Central Hospital from January 2022 to December 2022 was conducted.They were divided into a benign group(n=32)and a malignant group(n=88)based on the examination results.After admission,multi-slice spiral CT three-dimensional reconstruction technology was performed to compare the average CT value,peak enhancement,and CT sign detection rate between the two groups.The diagnostic efficacy of multi slice spiral CT three-dimensional reconstruction technology was observed based on the examination results.Results The average CT value and peak enhancement in the malignant group were higher than those in the benign group(P<0.05),and there was no statistically significant difference in the maximum diameter of the two groups of nodules(P>0.05).The detection rate of CT signs such as pleural indentation,ground glass sign,lobulation sign,vacuole sign,vascular bundle sign,spinous process sign,halo sign,and short hair spike sign in the malignant group was higher than that in the benign group,while the detection rate of calcification and satellite lesions was lower than that in the benign group(P<0.05).According to the examination results,the accuracy,sensitivity,and specificity of multi-slice spiral CT three-dimensional reconstruction technology in differential diagnosis of SPN were 95.83%,96.59%,and 93.75%,respectively.Conclusion The application of multi-slice spiral CT three-dimensional reconstruction technology in the differential diagnosis of benign and malignant SPN has high qualitative diagnostic value,which can comprehensively clarify the characteristics and morphology of the lesion,providing reference for effective clinical diagnosis and treatment.
