皮肤浅部真菌KOH镜检结果及细胞免疫相关指标对真菌感染和定植的鉴别效果分析

摘要 目的分析和研究皮肤浅部真菌KOH镜检结果及细胞免疫相关指标对真菌感染和定植的鉴别效果。方法选择2020年1月-2022年1月就诊的皮肤浅部真菌患者68例,取全部患者的待检样本,实施KOH镜检,评价KOH对各样本的阳性检出状况。此次的流式细胞仪为美国BectonDickinso公司生产,然后对细胞免疫指标CD3+、CD4+、CD8+水平予以检测,再对CD4+/CD8+比值进行计算,明确细胞免疫指标CD3+、CD4+、CD3+结果和CD4+/CD8+比值鉴别细胞感染、定植作用及效果。进一步采用ROC曲线分析细胞免疫相关指标CD3+、CD4+、CD8+和CD4+/CD8+比值鉴别细菌感染、定植效果。结果68例样本中KOH镜检提示51例阳性,阳性检出率为75.00%(51/68),其中体癣、足癣、甲癣、花斑、头癣样本阳性检出率分别为85.71%(12/14)、75.00%(9/12)、78.57%(11/14)、60.00%(9/15)、71.42%(10/14)。和真菌定植组比较,真菌感染组CD3+、CD4+、CD4+/CD8+下降,CD8+上升,差异有统计学意义(P<0.05)。结论运用KOH镜检对皮肤浅部真菌的阳性检出率高于60%,同时CD3+、CD4+、CD4+/CD8+于真菌感染中下降,CD8+在其中上升,能够用于真菌感染及定植的鉴别。 Objective To analyze and research differential effect of KOH microscopy Results and cellular immune related indicators on fungal infection and colonization of patients with superficial skin fungi deeply and in detail.Methods The paper chose 68 patients with superficial fungal skin in our outpatient clinic from January 2020 to January 2022,collected samples for testing and KOH microscopy to evaluate positive detection status of KOH on each sample.Flow cytometry was produced by BectonDickinson Company of the US,levels of cellular immune indicators CD3+,CD4+and CD8+were tested.CD4+/CD8+ratio was calculated to clarify Results of cellular immune indicators CD3+,CD4+,CD3+and CD4+/CD8+ratio in identifying cell infections,colonization and effect.Further,cell immunity related indicators CD3+,CD4+,CD8+and CD4+/CD8+ratio were analyzed with receiver operating characteristic to identify bacterial infection and colonization effect.Results Among 68 samples,51 were positive for KOH microscopy,with positive detection rate of 75.00%(51/68).Among them,positive detection rates for tinea corporis,tinea pedis,onychomycosis,macula and tinea capitis samples were 85.71%(12/14),75.00%(9/12),78.57%(11/14),60.00%(9/15)and 71.42%(10/14)respectively.Fungal infection group showed decrease in CD3+,CD4+,CD4+/CD8+and increase in CD8+compared with fungal colonization group,(P<0.05).Conclusion Positive detection rate of superficial skin fungi by KOH microscopy is higher than 60%.At the same time,CD3+,CD4+,CD4+/CD8+decrease in fungal infections,while CD8+increases,which can be applied for identification of fungal infections and colonization.
