血清25羟基维生素D、胱抑素-C、同型半胱氨酸与高血压合并骨质疏松症的相关性分析

摘要 目的 探讨血清25羟基维生素D[25-hydroxyvitamin D,25(OH)D]、胱抑素-C(cystatin C,Cys-C)、同型半胱氨酸(homocysteine,Hcy)与高血压合并骨质疏松症(osteoporosis,OP)的关系。方法 选取2019年10月至2022年10月北京积水潭医院贵州医院收治的185例单纯原发性高血压患者作为研究对象,根据是否合并OP划分为不伴OP组(103例)与伴OP组(82例)。另于同期选取本院80名体检健康者,设为对照组。比较3组研究对象临床资料差异,采用Pearson相关性分析各项指标与L2~4骨密度的关系,采用多元Logistic回归模型分析高血压并发OP的相关因素。结果 3组研究对象年龄、性别、三酰甘油(triacylglycerol,TC)比较差异无统计学意义(P>0.05)。3组研究对象体重指数(body mass index,BMI)、收缩压、舒张压、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、25(OH)D、Cys-C、Hcy、L2~4骨密度比较差异有统计学意义(P<0.05)。Pearson相关性分析显示,BMI与L2~4骨密度无明显相关性(P>0.05);HDL-C、25(OH)D与L2~4骨密度呈显著正相关(P<0.05);TG、Cys-C、Hcy与L2~4骨密度呈显著负相关(P<0.05)。多元Logistic回归分析显示,LDL-C、Cys-C、Hcy为高血压并发OP的独立危险因素(P<0.05),HDL-C、25(OH)D为高血压并发OP的保护因素(P<0.05)。结论 高血压合并OP患者血清25(OH)D、L2~4骨密度水平下降,Cys-C、Hcy水平升高。25(OH)D、Cys-C、Hcy与L2~4骨密度变化、高血压并发OP紧密相关。 Objective To investigate the levels of serum 25-hydroxyvitamin D [25(OH) D],cystatin C(Cys-C),homocysteine(Hcy) in patients with hypertension and osteoporosis(OP) and its relationship with hypertension and OP.Methods A total of 185 patients with essential hypertension admitted to Beijing Jishuitan Hospital Guizhou Hospital from October 2019 to October 2022 were selected as research objects.There were 103 patients with OP and 82 patients without OP.And 80 healthy people were selected as the control group.Clinical data were compared among the three groups.Pearson correlation was used to analyze the relationship between each index and L2-4 bone mineral density.Multiple logistic regression model was used to analyze the related factors of hypertension combined with OP.Results There were no differences in age,sex or triacylglycerol(TC) among the three groups(P<0.05).There were significant differences in body mass index(BMI),systolic blood pressure,diastolic blood pressure,high density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),25(OH)D,Cys-C,Hcy and L2-4 bone mineral density among the three groups(P<0.05).Pearson correlation analysis showed that there was no significant correlation between BMI and L2-4 bone mineral density(P>0.05),HDL-C and 25(OH)D were positively correlated with L2-4 bone mineral density(P<0.05),and TG,Cys-C and Hcy were negatively correlated with L2-4 bone mineral density(P<0.05).Multiple logistic regression analysis showed that LDL-C,Cys-C and Hcy were independent risk factors for hypertension complicated with OP(P<0.05),and HDL-C and 25(OH)D were protective factors for hypertension complicated with OP(P<0.05).Conclusion Serum 25(OH)D and L2-4bone mineral density are decreased,while the levels of Cys-C and Hcy are increased in OP patients with hypertension.25(OH)D,Cys-C and Hcy are closely related to the changes of L2-4 bone mineral density and hypertension complicated with OP.
