Monitoring report of nosocomial infections in a tertiary general hospital fr0m 2018 to 2022

摘要 目的分析某综合医院五年间医院感染病原菌。方法通过杏林医院感染实时监控系统收集2018-2022年该院医院感染病例信息,分析医院感染病原菌分布的情况包括病原菌组成,临床科室分布情况以及感染部位。结果2018-2022年累计监测住院患者101237例,发生医院感染2086例、2432例次,感染率为2.06%,感染例次率为2.40%;医院感染的高发科室为重症医学科、烧伤整形科、血液病科、呼吸内科、神经外科等;医院感染部位主要为下呼吸道、血液系统和泌尿系统;共培养分离病原菌2216株,其中革兰阴性菌1128株占50.90%,革兰阳性菌573株占25.86%,真菌515株占23.24%,排名前三位革兰阴性菌为肺炎克雷伯菌、大肠埃希菌、铜绿假单胞菌;前三位革兰阳性菌为凝固酶阴性葡萄球菌、金黄色葡萄球菌、屎肠球菌;前三位真菌为白假丝酵母、热带假丝酵母和光滑假丝酵母。结论医院感染防控应加强环境卫生,提高医务人员手卫生依从性,以减少医院感染的发生;重点关注患者下呼吸道感染和革兰阴性菌引起的感染。 OBJECTIVE To analyze the pathogenic bacteria of nosocomial infection in a general hospital over fiveyear period.METHODS The information on hospital infection cases in this hospital from 2018 to 2022 were collected through the real-time monitoring system of XINGLIN Nosocomial Infection Surveillance,and the distribution of hospital infection pathogens including the composition of pathogens,the distribution of clinical departments,and the site of infection were analyzed.RESULTS In From 2018-2022,a total of 101237 hospitalized patients were monitored cumulatively,of which 2086 cases and 2432 incidences of nosocomial infection occurred,with an infection rate of 2.06%and an infection case rate of 2.40%.The departments with high incidence of nosocomial infection were intensive care unit,department of burns and plastic surgery,department of hematology,department of respiratory medicine,department of neurosurgery,etc.The main sites of nosocomial infection were lower respiratory tract blood system and urinary system.A total of 2216 strains of pathogenic bacteria were cultured and isolated,of which 1128 strains of gram-negative bacteria accounted for 50.90%,573 strains of Grampositive bacteria accounted for 25.86%,and 515 strains of fungi accounted for 23.24%.The top three gram-negative pathogens were Klebsiella pneumoniae,Escherichia coli,and Pseudomonas aeruginosa;the top three grampositive bacteria were Staphylococcus aureus,Enterococcus faecium and Clostridium difficile;and the top three fungi were Candida albicans,Candida tropicalis and Candida glabrata.CONCLUSION It is suggested that hos-pital infection prevention and control should strengthen environmental hygiene and improve hand hygiene compliance among medical staff to reduce the occurrence of hospital infection.Attention should be paid to patients with lower respiratory tract infections and infections caused by gram-negative bacteria.
