早期综合干预对早产儿脑损伤综合征患者生长发育及神经系统后遗症的影响

Effect of Early Comprehensive Intervention on Growth,Development and Neurological Sequelae in Patients with Brain Injury Syndrome in Preterm Infants

摘要 目的:探析在早产儿脑损伤综合征患儿中实施早期综合干预对其生长发育、神经系统后遗症的影响,为临床提供经验。方法:回顾性分析2019年3月—2020年12月商丘市第一人民医院收治的100例脑损伤综合征早产儿的临床资料,根据干预方案的不同进行分组,将50例单一接受常规干预的早产儿作为对照组,50例在对照组基础上接受早期综合干预的早产儿作为观察组。观察两组早产儿发育情况、神经系统后遗症发生情况、智力发育商(DQ)与运动功能发育指数(PDI)评分情况。结果:干预12个月和24个月后,观察组早产儿身高高于对照组,且体重重于对照组,差异有统计学意义(t=3.896、3.724、10.595、9.340,P<0.05)。观察组神经系统后遗症发生率(6.00%)低于对照组(26.00%),差异有统计学意义(χ^(2)=7.440,P<0.05)。干预12个月和24个月后,观察组DQ、PDI较对照组高,差异有统计学意义(t=4.235、2.667、4.635、8.178,P<0.05)。结论:在脑损伤综合征早产儿中实施早期综合干预,可促进其生长发育,减少神经系统后遗症,有效提高运动功能。 Objective:To explore the impact of implementing early comprehensive interventions on growth,development,and neurological sequelae in children with brain injury syndrome in preterm infants,and to provide clinical experience.Methods:The clinical data of 100 preterm infants with brain injury syndrome admitted to the hospital from March 2019 to December 2020 were retrospectively analyzed,and 50 preterm infants who received a single conventional intervention were grouped according to the intervention protocol,and 50 preterm infants who received early comprehensive intervention on the basis of the control group were grouped as the observation group.The development of preterm infants,the occurrence of neurological sequelae,and the intellectual development quotient(DQ)and motor function development index(PDI)scores were observed in both groups.Results:After 12 and 24 months of intervention,the preterm infants in the observation group were taller and heavier than the control group,with statistically significant differences(t=3.896,3.724,10.595,9.340,P<0.05).The incidence of neurological sequelae in the observation group(6.00%)was lower than that in the control group(26.00%),and the difference was statistically significant(χ^(2)=7.440,P<0.05).After 12 and 24 months of intervention,DQ and PDI were higher in the observation group than in the control group,with statistically significant differences(t=4.235,2.667,4.635,8.178,P<0.05).Conclusion:Implementing early comprehensive interventions in preterm infants with brain injury syndrome can promote growth and development,reduce neurological sequelae,and effectively improve motor function.
