摘要
目的探讨认知心理护理配合以格林模式为基础的健康教育对老年急性脑梗死(AIS)患者负性情绪和睡眠质量的影响。方法选取2022年1—9月广东省东莞市滨海湾中心医院收治的老年AIS患者84例,采用随机数表法分为试验组(n=42)与对照组(n=42)。对照组接受常规护理,试验组在常规护理基础上接受认知心理护理及以格林模式为基础的健康教育,比较两组干预前后的疾病感知、负性情绪、睡眠质量。结果干预后试验组遵医行为97.62%高于对照组的78.57%,差异有统计学意义(P<0.05);试验组患者干预后的汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分较对照组低,差异有统计学意义(P<0.05);试验组患者干预后的简易版疾病感知问卷(BIPQ)评分较对照组高,差异有统计学意义(P<0.05);试验组患者干预后的匹茨堡睡眠质量量表(PSQI)评分较对照组低,差异有统计学意义(P<0.05)。结论认知心理护理配合以格林模式为基础的健康教育干预可提高老年AIS患者的疾病感知水平,改善患者遵医行为,调节患者负性情绪,改善睡眠质量,值得临床借鉴。
Objective To explore the impact of cognitive psychological nursing combined with health education based on the PRECEDE-PROCEED model on negative emotions and sleep quality in elderly patients with AIS.Methods A total of 84 elderly patients with AIS admitted to Dongguan Binhai Bay Central Hospital from January to September 2022 were divided into the test group(n=42)and the control group(n=42)using a random number table method.The control group received routine nursing,while the test group received cognitive psychological nursing and health education based on the PRECEDE-PROCEED model under routine nursing.The illness perception,negative emotions,and sleep quality of the two groups were compared before and after the intervention.Results The treatment compliance behavior of 97.62% of the test group after the intervention was higher than that of the control group 78.57%,with a statistically significant difference(P<0.05).The Hamilton anxiety scale(HAMA)score and Hamilton depression scale(HAMD)score of the test group after intervention were lower than control group,with a statistically significant difference(P<0.05).The brief illness perception questionnaire(BIPQ)score of the test group patients after the intervention was higher than that of the control group,with a statistically significant difference(P<0.05).The Pittsburgh sleep quality index(PSQI)score of the test group patients after the intervention was lower than that of the control group,with a statistically significant difference(P<0.05).Conclusion Cognitive psychological nursing combined with health education interventions based on the PRECEDE-PROCEED model can improve the illness perception level of elderly AIS patients,enhance their treatment compliance behavior,alleviate their negative emotions,and improve sleep quality,which is worthy of clinical reference.
