DSA设备不同曝光模式对冠状动脉造影辐射剂量及图像质量的影响分析 被引量:1

Analysis of effects of different exposure modes of DSA on radiation dose and image quality of coronary angiography

摘要 目的 探讨数字减影血管造影(digital subtraction angiography, DSA)设备不同曝光模式对冠状动脉造影辐射剂量及图像质量的影响。方法 回顾性分析2018年1月至2020年3月在河南省人民医院进行冠状动脉造影的184例患者。根据设备曝光模式的不同,共分为两组:冠状动脉造影模式(coronary angiography, CAG)组83例,应用标准曝光帧率及透视滤过(15 f/s、0.10 mmCu+1.00 mmAl);心脏电生理检查模式(cardiac electrophysiology, EP)组101例,采用降低曝光帧率、曝光加透视滤过[7.5 f/s、曝光滤过(0.10 mmCu+1.00 mmAl)、透视滤过(0.90 mmCu+1.00 mmAl)]。分析相关参数包括累积皮肤表面入射剂量(Cumulative Air Kerma, AK、mGy)、剂量面积乘积(Cumulative DAP,DAP、mGy·cm~2)、曝光时间(Fluo time, FT、min)、摄像帧数(FPS)、曝光系列数(Run)、性别、年龄、体重、身高及图像质量等。结果 两组辐射剂量AK、DAP值差异有统计学意义(Z=-10.160,P<0.001;Z=-10.071,P<0.001);曝光时间(Z=-1.816,P=0.069)、曝光序列数(Z=-0.249,P=0.803)、性别(χ^(2)=0.948,P=0.33)、年龄(t=0.709,P=0.479)、体重(t=-1.906,P=0.054)、身高(Z=-1.422,P=0.155)差异均无统计学意义(P>0.05);两组图像质量无明显差异,均能满足临床诊治要求。结论 冠状动脉造影采用心脏的电生理(EP)曝光模式与常规冠状动脉造影(CAG)曝光模式相比,在不影响图像质量的情况下,明显降低了患者和术者所受到的辐射剂量。 Objective To explore the effects of different exposure modes of DSA equipment on the radiation dose and image quality of coronary angiography.Methods Randomly collected 184 patients who underwent cardiovascular angi-ography from January 2018 to March 2020.According to the different exposure modes of the equipment,they were di-vided into two groups:Coronary Angiography(CAG)group including 83 cases,using standard exposure frame rate and fluoroscopy(15f/s,0.10mmCu+1.00mmAl),and 101 cases in the Cardiac Electrophysiology(EP)group,using re-duced exposure frame rate,exposure plus perspective filtering[7.5 f/s,exposure filtering(0.10mmCu+1.00mmAl),perspective filtering(0.90 mmCu+1.00 mmAl)].Analysis related parameters include cumulative skin surface incident dose(Cumulative Air Kerma,AK,mGy),dose area product(Cumulative DAP,DAP,mGy·cm^(2)),fluoroscopy time(Fluo time,FT,min),camera frame number(FPS),exposure series(Run),gender,age,body mass and image quality,etc.Results The differences inradiation dose AK and DAP values between the two groups were sta-tistically significant(Z=-10.160,P<0.001;Z=-10.071,P<0.001);number of exposure series(Z=-0.249,P=0.803),fluoroscopy time(Z=-1.816,P=0.069),gender(χ^(2)=0.948,P=0.33),age(t=0.709,P=0.479),body mass(t=-1.906,P=0.054),and height(Z=1.422,P=0.155)had no statistical differences(P>0.05);There was no significant difference in image quality between the two groups,and both could meet the clinical diagnosis cardiac electrophysiological exposure mode can significantly reduce the radiation dose of patients and operators without affecting the image quality.
