摘要
目的探讨3D打印技术辅助经椎弓根椎体截骨术治疗脊柱后凸畸形的临床效果。方法选取2019年4月至2022年1月收治的56例脊柱后凸畸形患者为研究对象,根据随机数字表法将其分为对照组(28例,经椎弓根椎体截骨术)与观察组(28例,3D打印技术辅助经椎弓根椎体截骨术)。比较两组的治疗效果。结果观察组的手术时间、住院时间短于对照组,术中出血量、术后引流量少于对照组(P<0.05)。术后3个月,观察组的Cobb角小于对照组,视觉模拟评分法(VAS)、Oswestry功能障碍指数(ODI)评分低于对照组(P<0.05)。术后1 d,两组的皮质醇(Cor)、去甲肾上腺素(NE)及C反应蛋白(CRP)水平高于术前,但观察组低于对照组(P<0.05)。观察组的手术并发症总发生率略低于对照组,但差异无统计学意义(P>0.05)。术后3个月,观察组的脊柱侧凸研究问卷表-22(SRS-22)各维度评分高于对照组(P<0.05)。结论3D打印技术辅助经椎弓根椎体截骨术可缩短脊柱后凸畸形患者的手术时间及住院时间,减少术中出血量及术后引流量,改善后凸畸形、疼痛程度及腰椎功能,提高生活质量。
Objective To investigate the clinical effect of 3D printing technology assisted transpedicular vertebral osteotomy in the treatment of patients with kyphotic deformity.Methods A total of 56 patients with kyphotic deformity admitted from April 2019 to January 2022 were selected as the research objects.According to the random number table method,the patients were divided into control group(28 cases,transpedicular vertebral osteotomy)and observation group(28 cases,3D printing technology assisted transpedicular vertebral osteotomy).The therapeutic effects of the two groups were compared.Results The operation time and hospitalization time of the observation group were shorter than those of the control group,and the intraoperative blood loss and postoperative drainage volume were less than those of the control group(P<0.05).At 3 months after operation,the Cobb angle of the observation group was smaller than that of the control group,and the scores of Visual Analogue Score(VAS)and Oswestry Disability Index(ODI)were lower than those of the control group(P<0.05).At 1 d after operation,the levels of cortisol(Cor),norepinephrine(NE)and C-reactive protein(CRP)in the two groups were higher than those before operation,but those in the observation group were lower than the control group(P<0.05).The total incidence of surgical complications in the observation group was slightly lower than that in the control group,but there was no significant difference between the two groups(P>0.05).At 3 months after operation,the scores of each dimension of Scoliosis Research Society-22(SRS-22)in the observation group were higher than those in the control group(P<0.05).Conclusion 3D printing technology assisted transpedicular vertebral osteotomy can shorten the operation time and hospitalization time of patients with kyphotic deformity,reduce intraoperative blood loss and postoperative drainage volume,ameliorate kyphosis,pain and lumbar function,and improve the quality of life.
