摘要
目的:观察通痹颗粒治疗类风湿关节炎的临床疗效及对铁调素、炎症细胞因子的影响。方法:将70例气血亏虚、痰瘀互结证的类风湿关节炎患者随机分为治疗组和对照组,每组35例。对照组予甲氨蝶呤口服,治疗组在对照组基础上加用通痹颗粒,疗程均为28天。对比两组患者的临床疗效、疼痛视觉模拟评分法(Visual Analogue Scale,VAS)评分、28个关节疾病活动度(Disease Activity Score in 28 Joints,DAS28)评分、红细胞沉降率、C反应蛋白等指标。结果:两组临床疗效比较,差异无统计学意义(P>0.05),但治疗组的总有效率高于对照组;治疗后两组患者VAS评分、DAS28评分和红细胞沉降率、C反应蛋白均较治疗前有所改善,铁调素的表达和炎症细胞因子白细胞介素-6、肿瘤坏死因子-α水平均较治疗前降低,治疗组优于对照组,差异具有统计学意义(P<0.05)。结论:通痹颗粒可以有效改善类风湿关节炎患者的临床症状体征,降低红细胞沉降率和C反应蛋白,并且对铁调素和炎症细胞因子白细胞介素-6、肿瘤坏死因子-α具有一定的调节作用。
Objective:To observe the clinical effect of the Tongbi granules(通痹颗粒)on rheumatoid arthritis(RA)and its influences on hepcidin and inflammatory cytokines.Methods:A total of 70 cases of RA patients with Qi(气)blood deficiency and phlegm blood stasis syndrome were randomly divided into the treatment group and the control group,with 35 cases in each group.O The control group was given methotrexate orally,and the treatment group was given the Tongbi granules on the basis of the treatment in the control group.The course of treatment was 28 days.The clinical efficacy,visual pain simulation score(VAS),disease activity score in 28 joints(DAS28),erythrocyte sedimentation rate and C-reactive protein were compared between the two groups.Results:There was no significant difference in clinical curative effect between the two groups(P>0.05).The total efficiency in the treatment group was higher than that in the control group,and the VAS,DAS28,erythrocyte sedimentation rate and C-reactive protein in the two groups were improved after treatment,while the hepcidin expression and the levels of interleukin-6 and tumor necrosis factor-αwere decreased,and the treatment group was better than the control group.Conclusion:The Tongbi granules can effectively ameliorate the clinical symptoms and signs of patients with rheumatoid arthritis,reduce erythrocyte sedimentation rate and C-reactive protein,and regulate hepcidin and interleukin-6 and tumor necrosis factor-α.
