摘要
目的探讨牙周牙髓联合病变患者根管治疗后不同时机牙周治疗的临床效果及对龈沟液炎症因子水平的影响。方法选取2021年1月至2022年5月新泰市人民医院收治的92例牙周牙髓联合病变患者作为研究对象,根据不同时机牙周治疗分为早期组(n=46)与延期组(n=46),早期组于根管治疗1周后给予牙周治疗,延期组于根管治疗5周后给予牙周治疗,比较两组临床疗效、龈沟液炎症因子水平及生命质量。结果早期组治疗有效率(91.30%)与延期组(80.43%)比较差异无统计学意义(P>0.05);治疗后两组龈沟液白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平降低(P<0.05),且早期组低于延期组(P<0.05);治疗后两组心理功能、生理功能、社会功能、日常活动评分升高(P<0.05),且早期组高于延期组(P<0.05)。结论不同时机对牙周牙髓联合病变患者进行牙周治疗均具有较高的临床疗效,但根管治疗后及早进行牙周治疗,能有效降低龈沟液炎症因子水平,提高生命质量。
Objective To explore the clinical effect of periodontal therapy at different time after root canal therapy in patients with periodontal pulp combined lesions and its influence on the level of inflammatory factors in gingival crevicular fluid.Methods A total of 92 patients with combined periodontal pulp lesions from January 2021 to May 2022 who were admitted in the Department of Stomatology of Xintai People's Hospital were selected,and divided into the early group(n=46)and the delayed group(n=46)by different times of periodontal treatment.The early group was given periodontal treatment after one week of root canal treatment,and the delayed group was given periodontal treatment after five weeks of oot canal treatment.The clinical efficacy,levels of inflammatory factors in gingival crevicular fluid and quality of life of the two groups were compared.Results There was no significant difference between early group(91.30%)and delayed group(80.43%)(P>0.05).After treatment,the levels of interleukin-1β(IL-1β),interleukin-6(IL-6)and interleukin-8(IL-8)in gingival crevicular fluid of two groups were decreased(P<0.05),and those in early group were lower than those in delayed group(P<0.05).After treatment,the scores of psychological function,physiological function,social function and daily activity of the two groups were increased(P<0.05),and the early group was higher than the delayed group(P<0.05).Conclusion Periodontal treatment for patients with combined periodontal pulp lesions at different times has a high clinical effect,but early periodontal treatment after root canal treatment can effectively reduce the level of inflammatory factors in gingival crevicular fluid and improve the quality of life.
